Full Job Description
The Division of Rehabilitation in the Department of Family Medicine is seeking a full-time associate. Joining a team of six other physicians who together provide care for approximately 90 patients, in collaboration with allied health, specialist colleagues and other partners. The expectations include daily visits during the week for their patients and to share on-call responsibilities with colleagues on the team. Experience managing hospital-based patients would be an asset. This is an excellent opportunity for a physician who wishes to complement their community practice with a component of hospital-based care. Remuneration is fee-for-service as set by the OHIP Schedule of Benefits and Fees. Accountabilities: • Deliver patient-centred rehabilitation services to individuals recovering from illness, injury, surgery, neurological, orthopaedic and other complex medical conditions • Collaborate with an interdisciplinary team to develop and implement individualized care plans that support medical health; patient recovery and functional goals; and contribute to effective team processes, including participation in team meetings, case conferences and care coordination activities • Assess, monitor and support patients in improving strength, mobility, independence and activities of daily living • Promote safe transitions across the continuum of care, including discharge planning and community reintegration • Participate in a collaborative, evidence-informed environment focused on quality patient outcomes and excellent patient experience • Support patients and families through education, goal setting and ongoing rehabilitation planning About the program: Osler's Rehabilitation Program delivers comprehensive inpatient rehabilitation services focused on maximizing patients' functional recovery, independence and quality of life. Through a collaborative, interdisciplinary approach, the program supports patients with a wide range of medical, neurological and orthopaedic conditions on their journey toward recovery and community reintegration. Qualifications • Must have a Certificate of Registration to practice from the College of Physicians and Surgeons of Ontario and have certification in Family Medicine from the College of Physicians of Canada (a Fellowship would be considered an asset) • Excellent communication skills in English (additional languages are considered an asset as we serve a highly diverse population) Additional Information Osler is a primary clinical partner for Toronto Metropolitan University School of Medicine and is an academic community partner with the University of Toronto's Temerty Faculty of Medicine and McMaster's Michael G. DeGroote School of Medicine. Successful applicants are required to participate in academic activities, which may include teaching, research or quality improvement initiatives. If the applicant does not currently hold an academic appointment with a medical school affiliated with the hospital, they must submit an application for a faculty appointment within three months of their hospital appointment.
