Full Job Description
Crouse's Pharmacy team is hiring a Director,Pharmacy Business Operations & 340B to provide strategic, financial, operational, and compliance leadership for Crouse Health's 340B Drug Pricing Program and retail pharmacy business. The Director is accountable for developing and optimizing the 340B infrastructure, leading the business operations and financial performance of retail pharmacy services, and advancing pharmacy strategies that improve patient access, prescription capture, and sustainable contribution margin. This role works across Finance, Pharmacy, Compliance, Legal, Revenue Cycle, Information Technology, Supply Chain, clinical operations, and external partners.

Pay Range: $145,000-$160,000 based on years of experience and education

Director, Pharmacy Business Operations & 340B Responsibilities:
  • Develops internal systems utilizing appropriate resources to support the operating needs of the assigned department(s) of responsibility including, but limited to: finance, planning, marketing, advertising, information systems, human resources and other support systems. Coordinates financial and budgetary acquisitions, allocations and utilization of resources needed for quality, cost-effective, program(s)/service(s).
  • Provides strategic guidance to leadership staff within all areas of responsibility.
  • Develops, plans, and evaluates short, mid and long-term goal setting and goal achievement through facilitation and guidance consistent with the strategic plan.
  • Maintains current industry knowledge serving as a subject matter expert; ensures compliance with all federal/state/local laws, regulatory/accrediting guidelines and professional standards within all areas of responsibility.
  • Attends and actively participates in hospital and performance improvement councils and committees as requested.
  • Proactively communicates with staff and keeps them informed on a regular basis or as needed; updates staff regarding policy changes in specialized areas.
  • Develops and utilizes department policy statements, scope of service, quality indicators and updates; revises as necessary.
  • Performs staff management functions related to employee selection, on-boarding/orientation, coaching/development, staffing, work assignments, performance management and goal alignment as guided by hospital policy and procedures and collective bargaining unit agreement (when applicable).
  • Participates, collaborates and actively engages in meetings to achieve goals and mutually beneficial outcomes.
  • Conducts regular staff meetings to disseminate hospital and/or department/division information and allows for discussion of staff concerns needing clarification or resolution.
  • Provide administrative leadership and day-to-day oversight of the health system's 340B Drug Pricing Program.
  • Maintain a comprehensive 340B governance framework, including policies, procedures, controls, work plans, committee reporting, and accountability across participating departments.
  • Coordinate HRSA/OPAIS registrations, annual recertification, covered entity and child-site information, and applicable pharmacy arrangements.
  • Oversee controls designed to prevent diversion and duplicate discounts and to maintain complete, accurate, and auditable records.
  • Monitor eligibility logic, accumulations, purchasing, replenishment, inventory, WAC exposure, carve-in/carve-out decisions, and exception resolution.
  • Lead internal monitoring, independent audits, HRSA audit readiness, corrective-action planning, and follow-through on identified issues.
  • Serve as the organization's primary business and operational subject-matter leader for 340B, escalating legal, regulatory, clinical, or compliance matters to the appropriate leaders.
  • Lead the business and administrative operations of the health system's retail pharmacy, including launch planning, operating workflows, staffing models, budgeting, productivity, vendor coordination, and performance improvement.
  • Develop and execute strategies to increase appropriate prescription capture from hospital discharges, emergency department visits, ambulatory practices, employees, and other eligible channels.
  • Partner with the PIC to implement and optimize meds-to-beds, discharge prescription services, delivery, employee pharmacy offerings, and other patient-access initiatives.
  • Evaluate and develop phased opportunities for specialty pharmacy, mail or delivery services, ambulatory pharmacy programs, contract pharmacy relationships, and other service-line growth.
  • Ensure business initiatives and operating workflows are coordinated with pharmacy licensure requirements, patient-safety standards, and clinical policies.
  • Own the annual operating and capital budget process for assigned 340B and retail pharmacy operations and monitor actual performance against budget and forecast.
  • Develop dashboards and routine reporting for 340B savings, prescription volume, capture rate, payer mix, reimbursement, acquisition cost, gross margin, labor productivity, compliance exceptions, and contribution margin.
  • Identify the operational drivers of performance, recommend corrective actions, and hold internal and external partners accountable for results.
  • Build business cases, financial models, implementation plans, and executive recommendations for pharmacy expansion and optimization opportunities.
  • Partner with Finance and Revenue Cycle to ensure accurate accounting, reconciliation, payment, reimbursement, and management reporting processes.
  • Manage business relationships with 340B third-party administrators, split-billing vendors, wholesalers, manufacturers, pharmacy benefit managers, payers, contract pharmacies, consultants, and other partners.
  • Lead or support vendor selection, due diligence, contracting, implementation, service-level monitoring, issue escalation, and performance reviews.
  • Coordinate with Legal, Compliance, Supply Chain, and Finance to evaluate contract terms, regulatory considerations, financial impact, and operational risk.
  • Promote effective collaboration with hospital departments, medical practices, employed providers, community partners, and external pharmacies.
  • Develop and maintain a multiyear pharmacy business strategy aligned with Crouse Health's financial, clinical, patient-access, and market-growth objectives.
  • Chair or coordinate the 340B Oversight Committee, establish agendas and decision logs, and provide regular reporting to senior leadership.
  • Translate complex program, financial, and operational information into clear recommendations, priorities, and action plans.
  • Lead cross-functional projects from concept through implementation, including change management, stakeholder communication, education, and post-launch optimization.
  • Maintain current knowledge of the evolving 340B, retail pharmacy, payer, reimbursement, and healthcare regulatory environment and coordinate organizational responses with subject-matter experts.
  • Foster a culture of compliance, accountability, service, continuous improvement, and responsible stewardship.


Director, Pharmacy Business Operations & 340B Requirements:
  • Bachelor's degree in business administration, healthcare administration, finance, pharmacy administration, public health, or a related field. Master's degree preferred
  • Five (5) to seven (7) years of progressively responsible experience in healthcare operations, pharmacy business operations, 340B, finance, revenue cycle, compliance, or a closely related field.
  • Strong financial, analytical, project-management, and problem-solving capabilities, including the ability to interpret complex data and convert findings into action
  • Experience managing vendors, contracts, implementations, or outsourced service relationships.
  • Effective written, verbal, and executive-level communication skills.
  • High level of integrity, sound judgment, attention to detail, and commitment to regulatory compliance.
  • Direct experience leading or administering a hospital or health-system 340B program.
  • Retail, ambulatory, specialty, or health-system pharmacy business experience.

Preferred:
  • Experience with 340B split-billing, accumulator, or third-party administrator platforms and pharmacy wholesaler processes.
  • Experience with retail pharmacy reimbursement, payer or PBM contracting, prescription-capture strategy, or meds-to-beds programs.
  • 340B ACE certification at hire or ability to obtain within 12 months of appointment.
