Description

Hospice of Orange & Sullivan is seeking an experienced and compassionate Licensed Clinical Social Worker (LCSW) to lead our Social Work and Bereavement Care teams. This leadership role is responsible for directing, coordinating, and supporting interdisciplinary hospice services while ensuring the delivery of exceptional patient- and family-centered care.

The Director provides clinical and operational oversight of social work and bereavement services, supervises team members, participates in interdisciplinary care planning, ensures regulatory compliance, and drives quality improvement initiatives. This position plays a vital role in supporting patients, families, caregivers, and staff throughout the hospice journey while fostering excellence in clinical practice and customer service.
Key Responsibilities
  • Provide leadership and supervision to Social Work and Bereavement Care teams
  • Co-facilitate interdisciplinary group (IDG) meetings and support care coordination
  • Ensure compliance with hospice regulations, accreditation standards, and organizational policies
  • Mentor, coach, and develop team members through education, performance management, and professional development
  • Oversee bereavement program development, evaluation, and enhancement
  • Participate in quality improvement initiatives and organizational leadership activities
  • Collaborate with community partners and serve as Chair of the Veteran-to-Veteran Program Committee
  • Co-facilitate the training and onboarding of new volunteers
