Qualifications

Responsibilities

• Assess residents' social, emotional, and psychological needs to develop care plans.
• Provide counseling and support to residents and their families as needed.
• Coordinate discharge planning and community resources effectively.
• Maintain accurate and confidential resident records consistently.
• Participate in interdisciplinary team meetings for comprehensive care.
• Ensure compliance with all relevant regulations for social services in a nursing home.
• Supervise and train social services staff to uphold quality standards.
• Advocate for residents and address their concerns promptly.
