3200 G Street, South Sioux City, NE 68776Continental Springs Rehabilitation Center is seeking an experienced and motivated Director of Rehabilitation to lead our in-house rehabilitation program.
This is an excellent opportunity for a rehabilitation professional with a PT, OT, SLP, COTA, or PTA background who is passionate about leadership, clinical excellence, team development, and improving outcomes for residents.
The Director of Rehabilitation will oversee the daily operations of the rehabilitation department, support therapy staff, coordinate departmental activities, and work closely with nursing and other members of the interdisciplinary team.

Responsibilities
- Lead and oversee the day-to-day operations of the rehabilitation department.
- Provide leadership, direction, and support to physical, occupational, and speech therapy staff.
- Coordinate therapy schedules, staffing, productivity, and coverage.
- Monitor resident progress, clinical outcomes, quality of care, and achievement of therapy goals.
- Ensure therapy services are provided according to physician orders, plans of care, professional standards, and applicable regulations.
- Collaborate with therapists, nursing, physicians, social services, administration, residents, and families to support resident-centered care.
- Participate in care plan meetings and interdisciplinary team meetings.
- Monitor therapy documentation for accuracy, timeliness, completeness, and compliance.
- Support compliance with Medicare, Medicaid, state and federal regulations, payer requirements, and facility policies.
- Assist with recruitment, onboarding, orientation, training, mentoring, and performance management of rehabilitation staff.
- Promote teamwork, accountability, professional development, and strong communication within the rehabilitation department.
- Monitor departmental productivity, staffing, quality indicators, and operational goals.
- Identify opportunities to improve resident outcomes, functional independence, satisfaction, and departmental efficiency.
- Maintain appropriate staffing levels and assist with scheduling and coverage when necessary.
- Support survey readiness, audits, and regulatory compliance activities.
- Maintain current knowledge of rehabilitation practices, documentation requirements, reimbursement guidelines, and industry standards.
- Promote a culture of safety, dignity, respect, and compassionate resident care.
- Perform other duties as assigned by facility leadership.
QualificationsRequired rehabilitation background:Candidates must have a professional rehabilitation background as a:
- Physical Therapist (PT)
- Occupational Therapist (OT)
- Speech-Language Pathologist (SLP)
- Certified Occupational Therapy Assistant (COTA)
- Physical Therapist Assistant (PTA)
Additional qualifications include:- Current, valid Nebraska license or certification appropriate to the candidate's clinical discipline and in good standing.
- Previous rehabilitation leadership, supervisory, or management experience preferred.
- Skilled nursing facility (SNF), subacute, post-acute, or long-term care experience strongly preferred.
- Strong knowledge of rehabilitation practices and clinical documentation.
- Knowledge of Medicare, Medicaid, therapy documentation, and reimbursement requirements.
- Strong leadership, communication, organizational, and problem-solving skills.
- Ability to manage staff and coordinate daily departmental operations.
- Strong understanding of care planning and regulatory compliance.
- Experience with electronic medical record systems preferred; Net Health experience is a plus.
- Ability to work effectively with residents, families, therapists, nursing staff, physicians, and facility leadership.
