Home: Heartwood Long Term Care

Title: Director of Quality & Risk

Location: Cornwall, Ontario

Salary Range: $85,000.00 - $95,000.00

Status: Full-time, Vacancy

Reporting to the Director of Care, the Director of Clinical Services supports the Director of Care in managing all aspects of nursing services within the home. The Director of Clinicals Services ensures the delivery of high-quality resident care by promoting best practices, maintaining compliance with legislative and budgetary requirements, and supporting an interdisciplinary approach to care.

This is a full-time position based in a professional office environment. Standard working hours are Monday through Friday, 8:00 - 4:00 with occasional need for overtime during peak periods. The role involves prolonged periods of sitting and working on a computer. The employee must be able to manage multiple tasks and deadlines in a fast-paced, team-oriented setting.

Key Responsibilities:
  • Assist the Director of Care in implementing and evaluating quality improvement initiatives, risk management systems, data collection and analysis, report writing, and procedural updates within the nursing department.
  • Lead the coordination and implementation of best practice and risk management programs related to resident care (e.g., falls prevention, restraint reduction, wound management, infection control).
  • Monitor and manage the effective utilization of nursing supplies and resources.
  • Oversee the High Intensity Needs (HIN) program and ensure appropriate resident support and documentation.
  • Maintain systems for resident care assessment, planning, implementation, and evaluation at both the resident and unit levels, maximizing staff resources and supporting Southbridge Pinewood's interdisciplinary approach.
  • Support and oversee the RAI-MDS program to ensure accuracy and compliance.
  • Maintain an active, visible presence in resident home areas, mentoring staff and monitoring the provision of quality care.
  • Investigate and implement corrective actions related to occupational health and safety; develop and monitor modified work programs for nursing staff as required.
  • Collaborate with the Director of Care on special projects, reviews, audits, and other assigned duties.
  • Assume responsibility for the nursing department in the absence of the Director of Care.


Knowledge and Skills Required:
  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO)
  • Minimum of three years of experience in long-term care; previous management or leadership experience is an asset.
  • Strong knowledge of geriatric care, infection prevention and control, adult education principles, and quality improvement processes.
  • Proficient in RAI-MDS and computerized resident care systems.
  • Demonstrated leadership, organizational, and communication skills (both verbal and written).
  • Proficiency in Microsoft Word, Excel, and other clinical software applications.
  • Knowledge of applicable legislation, current nursing standards, and healthcare practices, with a demonstrated commitment to providing quality resident-centered care.
  • Flexibility to participate in the manager on-call rotation schedule. More information will be provided during the interview process.
  • A negative vulnerable sector check completed within 6 months of hire date.
  • Completion of a 2 Step TB Skin test within the last 12 months.
