  • Coordinate with physicians and other providers for optimal clinical outcomes
  • Communicate regularly with families about care and medical issues

Job Requirements

  • Ability to communicate effectively in English; both oral and written


Job Responsibilities Include

  • The professional knowledge and skills necessary to plan, organize develop and direct the overall operation of the Resident Care Department in accordance with the current state, federal and local standards to insure that the highest degree of quality care is maintained at all times
  • Qualified by experience and training to assess resident needs and to develop a service care according to these needs. Lead,develop, and support nursing team, med techs and care staff through rolemodeling and the company culture
  • Leadthe community medication management and clinical quality assurance programs
  • Coordinatewith physicians, rehab, and other select preferred providers to promote positiveclinical outcomes
  • Communicate regularly and timely with families regarding care and medical issues via phone, email or care plan conferences


Qualifications:

  • RN/LPN preferred but not required
  • Two or More years of supervisory experience and One Year in Assisted Living/Personal Care preferred
