Primary Responsibilities of the Director of Memory Care

Operations & Department Management
  • Represent the Memory Care Neighborhood in daily staffing meetings, ensuring effective communication between wellness and memory care employees across all shifts
  • Maintain compliance with state and federal regulations; participate in regulatory surveys and reviews as requested, and respond to state requests for information within required time limits, notifying leadership of any agency requests.
  • Adhere to the Resident Bill of Rights and promote dignity and respect among all residents and caregivers.
  • Lead and/or attend in-service and monthly staff meetings, and meet all state training requirements for the position.
  • Maintain working knowledge of fire and disaster plans and individual responsibilities under emergency conditions, reporting to work per schedule and company policies, including during severe weather.
  • Continuously expand knowledge of activity programs for older adults with moderate to severe memory impairments in a long-term care setting.
  • Maintain an organized filing system for the Memory Care Department and control expenses for activity supplies, durable medical equipment, and engagement vendors.


Resident Services & Continuity of Care

  • Accomplish all duties observing appropriate safety practices, and delegate tasks to qualified staff according to state law and regulations.
  • Recognize changes in residents' conditions, report promptly to the Director of Wellness, communicate with families as appropriate, and notify the physician per company policy.
  • In collaboration with the Director and Assistant Director of Wellness, develop personalized service plans reflecting each resident's preferences; document care, monitor health status, ensure social worker services are provided, and connect families with information and support groups.
  • Manage resident move-ins, advocacy meetings, and care plan meetings with residents and families per state and company guidelines.
  • Deliver a structured, daily activity program that supports joy, compassion, and creativity, and is appropriate for persons with Dementia and/or Alzheimer's Disease. Lead, train, and assign activity programs to Personal Assistants per shift, including delivery of our exclusive Reunion program under the direction of the Director of Wellness.
  • Oversee the monthly Memory Care calendar, ensuring all aspects of the QSLM "Walk With Me" program are utilized daily.
  • Using the iN2L program, ensure proper daily usage, including activities and one-on-one profiles built around individual residents' needs.
  • Manage monthly family communication-including newsletters with prior-month activities, photos, and upcoming events-per QSL Management expectations.
  • Utilize Memory Care progress notes and communication forms effectively and in a timely manner.
  • Provide training and orientation for new employees, monitor completion of monthly computerized training, and ensure all staff complete required Alzheimer's care training with documentation maintained in personnel files.
  • Consult with wellness leadership to determine whether treatments require external resources (home health, outpatient rehab, hospice, DME) or discharge to a higher level of care.
  • Observe for and immediately respond to any suspected, alleged, or actual resident abuse, neglect, or exploitation according to company policy and state regulations.
  • Coordinate elopement drills per state regulations, promote resident safety, and implement fall prevention programs.
  • Provide follow-up, intervention, and supervision for residents with a recent change in condition, documenting all actions taken.
  • Promote and assist residents participating in memory enhancement and performance improvement programs.
  • Ensure resident charts are current and organized, and maintain a clean environment that promotes well-being.
  • If a licensed nurse, assist with documentation of resident weights per service plans and physician records; under the direction of the Director of Wellness, oversee and perform medication pass as needed and train new nurses, med-aides, and med-techs per state and company expectations.

Education & Experience:

To perform this job successfully, the individual must be able to complete each essential duty satisfactorily, have excellent communication skills, interact effectively with residents and community personnel, and be able to maintain a high level of confidentiality. The Director of Memory Care must either:

  • Meet all state and federal regulations for assuming the position of a Licensed Practical Nurse and/or a Registered Nurse at an assisted living facility and be in good standing with the Board of Nursing, or
  • Be a Certified Dementia Care Practitioner and/or Montessori Dementia Care Practitioner with congregate care leadership experience or
  • Be a recreational therapist, social worker, or care manager with direct experience in positive management techniques for addressing challenging behaviors related to dementia.?


  • A caring heart and genuine desire to serve memory-impaired seniors
  • LPN certification required
  • Minimum of 2 years' experience working with seniors, preferably in memory care, long-term care, or healthcare settings
  • High School Diploma or GED required
  • Working knowledge of Microsoft Word, PowerPoint, and Publisher for calendar creation
  • Must have and maintain a safe driving record
  • Skills in building life stations, craft planning, design, creativity, and innovative activity development are required
