Success in this role requires a strong understanding of the hospice and post-acute care continuum, demonstrated healthcare leadership experience, exceptional relationship-building skills, and the ability to translate partnerships into measurable growth and improved patient outcomes. The Director will monitor partnership performance, analyze key metrics, forecast market and referral trends, and provide leadership with actionable insights to inform strategic decisions and future growth.

The ideal candidate is a mission-driven healthcare leader and strategic relationship builder who can establish credibility with hospital executives, physicians, case management teams, and clinical leaders while representing the organization with professionalism, compassion, and a commitment to delivering meaningful value to every partner and patient served.
