RESPONSIBILITIES:- Lead and monitor Quality Assurance and Performance Improvement (QAPI) initiatives throughout the facility
- Conduct clinical audits to ensure compliance with CMS, NYSDOH, federal regulations, and facility policies
- Track, trend, and analyze quality indicators including falls, infections, rehospitalizations, pressure injuries, and other clinical outcomes
- Collaborate with nursing leadership and interdisciplinary teams to develop and implement corrective action plans
- Identify opportunities for process improvement and support implementation of best practices
- Assist with survey readiness activities and support regulatory inspections and audits
- Educate and mentor staff on quality measures, documentation standards, regulatory requirements, and clinical best practices
- Participate in root cause analyses and performance improvement projects
- Monitor quality metrics and provide recommendations to improve resident outcomes and operational performance
- Maintain accurate records, reports, and quality improvement documentation
- Perform additional duties as assigned
REQUIREMENTS:- Minimum education requirement: Associate's Degree in Nursing; Bachelor's Degree in Nursing preferred
- Minimum experience requirement: Long-term care or skilled nursing experience preferred
- Current New York State Registered Nurse (RN) License required
- Strong knowledge of CMS regulations, NYSDOH requirements, QAPI principles, and the survey process
- Experience with clinical auditing, data analysis, and quality improvement initiatives
- Excellent communication, organizational, leadership, and problem-solving skills
- Ability to analyze data, identify trends, and implement performance improvement strategies
- Strong attention to detail and commitment to regulatory compliance and resident-centered care
- Ability to collaborate effectively with interdisciplinary teams and facility leadership
