Position Summary
TheDirector of Clinical Programs oversees and manages key clinical programs,including School-Based Health, Chronic Care Management (CCM), Annual WellnessVisits (AWV), and Transitional Care Management. This role is responsible forensuring high standards of patient care, regulatory compliance, programperformance, and efficient service delivery.
The Directorof Clinical Programs collaborates closely with the Chief Medical Officer toalign program goals with organizational priorities. This position overseesprogram staff, supports recruitment and employee development, monitorsproductivity and program outcomes, and ensures accurate and timely reporting.
Essential Duties andResponsibilities
Clinical Program Oversight
• Oversee School-Based Health programs, includingprogram outcomes, community outreach, participation, resources, and requiredreporting.
• Manage the Chronic Care Management (CCM) programand ensure appropriate care management for patients with chronic conditions.
• Ensure CCM documentation and billing practicescomply with Medicare and applicable regulatory requirements.
• Manage the Annual Wellness Visit (AWV) program,including pre-visit planning, patient outreach, preventive care, productivity,and daily operations.
• Oversee Transitional Care Management services tosupport patients transitioning from hospital to home and promote timelyfollow-up and continuity of care.
• Collaborate with multidisciplinary teams toaddress patient needs, improve outcomes, and reduce avoidable readmissions.
• Monitor program metrics, productivity, patientoutcomes, and regulatory requirements to identify and implement opportunitiesfor improvement.
Staff Management & Operations
• Supervise, mentor, and support assigned clinicalprogram staff, including School-Based Health, Telehealth, and Parents asTeachers employees.
• Provide staff with guidance, performancefeedback, and professional development opportunities.
• Support recruitment and onboarding of employeesfor assigned clinical programs.
• Develop and implement strategies to improveprogram operations, productivity, service delivery, and resource utilization.
• Conduct regular program and staff meetings toaddress performance, workflow, and operational needs.
• Foster effective communication and collaborationamong program staff, providers, departments, and external partners.
• Represent HCPSC at meetings, conferences, andcommunity events as required.
• Perform other duties as assigned.
Education & Experience
• Completion of an accredited nursing programrequired; Bachelor's degree in Nursing or higher preferred.
• Current multistate Registered Nurse (RN) orNurse Practitioner (NP) license required.
• Minimum of three (3) years of experience inclinical program management or a related healthcare leadership role.
• Knowledge of Chronic Care Management (CCM),Annual Wellness Visits (AWV), Transitional Care Management, and applicableregulatory requirements.
• Experience supervising and developing healthcarestaff preferred.
• Experience with Electronic Health Record (EHR)systems and healthcare technology required.
• Valid driver's license and access to reliabletransportation for in-state travel required.
Knowledge, Skills, and Abilities
• Strong leadership, organizational,communication, and staff management skills.
• Knowledge of clinical program operations,regulatory requirements, care coordination, and medical terminology.
• Ability to analyze productivity, programoutcomes, and performance measures and implement improvements as needed.
• Strong problem-solving and decision-makingskills with the ability to manage multiple programs and priorities.
• Ability to collaborate effectively withpatients, providers, employees, leadership, and community partners.
• Proficiency in Electronic Health Record (EHR)systems and other relevant healthcare software.
• Ability to maintain confidentiality and protectpatient information in accordance with HIPAA and organizational policies.
