Full Job Description
As Director, Integrated Care Solutions, you will be the vital connector between hospital and community partners and our internal teams-orchestrating coordinated planning, resolving complex challenges, navigating governance with confidence, and uncovering exciting opportunities for growth and continuous improvement. This is where strategy meets execution, and where your leadership directly influences the future of integrated health care delivery.

Annual salary is between $120,000 - $140,000 per year. Compensation is based on experience. Performance bonuses are also available.

Job Description
  • Build and maintain strong relationships with hospitals, community organizations, and key stakeholders to identify and advance partnership opportunities aligned with organizational and health system priorities.
  • Support the planning and implementation of strategic initiatives (e.g., Hospital-to-Home programs, integrated care pathways) in collaboration with operational teams, corporate functions, and external partners.
  • Facilitate cross-functional collaboration to ensure initiatives remain aligned, risks and barriers are identified, and drive progress towards overall objectives.
  • Support governance structures and stakeholder engagement activities by coordinating collaborative forums and ensuring timely follow-through on decisions.
  • Identify risks, barriers, and opportunities, leveraging data and stakeholder insights to support resolution, escalation, and continuous improvement.
  • Develop proposals and business cases that advance organizational and health system priorities.


Qualifications
  • A degree in Health Administration, Health Sciences, or a clinical health-related discipline
  • Master's degree in Business, Health Administration, or a related field is considered an asset
  • Ten (10) years of recent related experience in progressively responsible leadership positions within healthcare environments
  • Designation as a Regulated Health Professional and membership in good standing with the relevant professional college or association is considered an asset
  • Canadian Health Executive (CHE) certification is considered an asset
  • Strong leadership experience working across healthcare systems, including hospitals, community, and integrated care models
  • Demonstrated success supporting partnership growth, strategic initiatives, and organizational priorities
  • Strong knowledge of Ontario's health system design, funding models and an understanding of priority issues facing local/regional health systems
