Directs, implements and manages the goals, objectives and policies of the department ensuring adherence to all home health service organization's policies and procedures. Conducts supervisory home care visits and facilitates patient education programs.
Job Responsibility
Leads a Home Care Services group by communicating with and developing staff members, and building consensus for programs and goals that support a business, function or geographic area.
Develops and articulates a short-term strategic vision for areas of responsibility.
Participates in development of and adherence to all home health service organization's policies, procedures and protocols.
Supervises and monitors patient care activities to ensure compliance with the goals and plan of treatment.
Conducts supervisory home care visits to ensure that safe, effective care is being provided.
Assesses staff competency and proficiency; identifies staffing needs.
Ensures completion of appropriate documentation to facilitate reimbursement.
Directs the recruitment, orientation, training and management of progression staff; performs annual reviews on all direct report personnel.
Counsels and disciplines staff, when necessary.
Supervises and facilitates all patient education training programs.
Participates in the development, implementation and evaluation of the Quality Improvement Program.

Job Qualification
Masters degree in Nursing or Doctorate in Physical Therapy or a related field.
Current license to practice as a Registered Professional Nurse or Physical Therapist in New York State required, plus specialized certifications as needed.
Minimum five (5) years progressive responsiblity related to clinical and management aspects of a Certified Home Health Agency, required.
