Overview

Director, Clinical and Home Care Services

Responsible for the overall management and coordination of clinical services provided in the center and in the participant home. Assures compliance with all state and federal regulations. Trains and supervises the LIFE clinical and caregiver staff to ensure regulatory compliance and quality while maintaining an optimal level of independence for participants. Provides technical, organizational, and interpersonal skills necessary to effectively and efficiently coordinate the scheduling, communication and documentation of all aspects of the care plan. Responsible for assessment of health needs, creation and implementation of nursing care plans and providing nursing care, as well as, periodic re-evaluation of individual nursing needs. Demonstrates the knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants according to assigned responsibilities and LIFE standards.

Qualifications
