POSITION OVERVIEW

Job Summary
The Director of Value-Based Care Operations leads the execution and growth of all value based care programs, ensuring strong clinical outcomes, financial performance, and operational excellence across the organization. It drives enrollment, strengthens provider and partner operations, and manages key relationships with health plans, ACOs, and clinical leaders. Through cross functional leadership, training, and performance management, the role ensures value based models are adopted effectively and deliver measurable impact.

Roles and Responsibilities
• Leads and supports end to end project management activities, including project planning, timelines, stakeholder coordination, risk identification, and progress tracking to ensure on time and accurate delivery.
• Partners with cross functional stakeholders to drive projects from concept through implementation, ensuring scope alignment, clear communication, and effective issue resolution throughout the project lifecycle.
• Leads the operational execution of all value based care programs, including I SNP/IE SNP, ACO partnerships, and GUIDE/TEAM initiatives.
• Identifies and implements new value based partnerships and economic models to expand organizational participation and financial opportunity.
• Serves as the internal subject matter expert on value based care models, reimbursement structures, and program requirements.
• Designs and scales lead generation strategies to drive enrollment growth across I SNP and related programs.
• Orchestrates campus driven enrollment efforts, including sales support, scripting, and workflow optimization to improve conversion rates.
• Monitors enrollment pipelines and drives accountability for growth targets across campuses.
• Owns clinical and financial performance outcomes, including hospitalizations, LOS, quality metrics, and total cost of care.
• Partners with clinical and operational leaders to improve outcomes and implement corrective action plans.
• Tracks and reports financial performance, including shared savings and PMPY impact, to inform strategic decisions.
• Oversees provider operations, including NP and Medical Director setup, performance, and ongoing support.
• Manages relationships with health plans, ACO operators, and hospital systems to strengthen collaboration and referral pathways.
• Supports contracting, onboarding, and implementation for all value based programs and new campus or provider participants.
• Ensures compliance with program requirements, documentation standards, and reporting expectations.
• Develops and delivers training and change management programs to drive adoption of value based workflows and best practices.
• Other duties as assigned.

Qualifications
Education: Bachelor Degree
Experience: 5-8 years

Licenses and Certifications
MBA degree preferred.
Project Management Professional (PMP) certification preferred.

Physical Requirements
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
