Responsibilities

• Collaborate with the medical team on patient care concerns, hospitalizations, and discharges.
• Coordinate home training sessions and patient care conferences efficiently.
• Monitor patient supply ordering and usage while upholding care quality.
• Communicate ongoing program activities with the care team regularly.
• Monitor patient care parameters continuously for safety.
• Perform all registered nurse functions as required.
• Maintain thorough records and documentation according to regulatory standards.
