Provides diabetes education for inpatients and outpatients through private sessions and group classes. Monitors patient progress and provides ongoing support and education. Collaborates with healthcare professionals to ensure comprehensive diabetes care. Stays updated on best practices in diabetes management and wellness programming. Participates in community outreach to promote diabetes awareness and wellness initiatives. Promotes wellness initiatives to improve patient outcomes related to diabetes. Engages in diabetes management activities, including monitoring and support. Creates personalized education plans for diabetes management. Accurately documents patient information related to diabetes care. Provides staff training on diabetes care and treatment. Other duties as assigned.
