Delegating Nurse

As Delegating Nurse, you'll play an essential role in enriching the lives of seniors by providing clinical leadership, coordinating care, and supporting high-quality resident outcomes every single day.

Your Role:

As the Delegating Nurse, you assist with the supervision of the nursing department while serving as a clinical leader and resource for resident care. You provide oversight, education, and coordination of services to ensure residents receive safe, appropriate, and person-centered care across the community. This role partners closely with community leadership and clinical teams to support regulatory compliance, staff development, and continuous quality improvement.

  • Status: FULL TIME
  • Schedule: Sunday -Thursday 7am-3
  • Location: Brooklyn Heights
  • Rate of Pay: $110,000.00 Annual Salary

What You'll Do:
  • Advocate for resident-centered care through care coordination, case management, and clinical oversight.
  • Serve as a liaison with external providers and partners including physicians, pharmacies, hospice, home health, therapy services, and families.
  • Complete resident nursing assessments upon admission, re-admission, and at established intervals or with significant change in condition.
  • Conduct self-medication assessments and ongoing medication management oversight as required.
  • Maintain and update resident assessment tools and clinical documentation to support accurate service planning.
  • Provide education and training to team members related to resident care needs, service plans, and clinical best practices.
  • Support clinical oversight of resident care across applicable care settings within the community.
  • Review, sign, and follow up on incident and accident reports within required timeframes, ensuring appropriate interventions are implemented.
  • Partner with the Director of Health & Wellness and leadership team to supervise nursing staff, including coaching, scheduling, performance management, orientation, and staff development.
  • Encourage team engagement and participation in employee satisfaction initiatives and support action planning for continuous improvement.
  • Participate in marketing, admission, and care planning meetings as needed.
  • Prepare for and participate in Quality Assurance and performance review meetings, including audits and pharmacy reviews.
  • Review and update nursing policies and procedures in collaboration with community leadership.
  • Support medication administration training, refresher courses, and competency validation.
  • Conduct ADL competency assessments and medication pass reviews.
  • Participate in on-call rotation and provide floor coverage as needed to support operations.
  • Perform other duties as assigned

Supervisory Responsibilities:
  • Provide supervision and leadership to nursing and care team members as assigned.
  • Carry out supervisory responsibilities in accordance with organizational policies and applicable regulations.
  • Participate in interviewing, hiring, training, scheduling, performance evaluation, coaching, corrective action, and issue resolution.
  • Promote a positive, supportive, and accountable work environment.

Qualifications:
  • Current Registered Nurse (LPN) license in good standing.
  • Current CPR and First Aid certification.
  • Previous experience in senior living, long-term care, or a similar healthcare environment preferred.
  • Strong leadership, communication, and organizational skills.
  • Ability to coordinate care across interdisciplinary teams and external providers.
  • Ability to anticipate clinical needs, apply sound judgment, and manage confidential information with discretion.
  • Proficiency with clinical documentation systems and basic computer applications.
  • Ability to work a flexible schedule based on community needs.
