What You'll Do- Evaluate and treat speech, language, cognitive-communication, and swallowing disorders in patients' homes.
- Develop individualized treatment plans that support meaningful functional goals.
- Educate caregivers and family members to reinforce therapy between visits.
- Collaborate with physicians, nurses, physical therapists, occupational therapists, and the interdisciplinary care team.
- Monitor patient progress and adjust interventions as appropriate.
- Maintain accurate and timely clinical documentation.
What We're Looking For- Active Minnesota Speech-Language Pathologist (SLP) license.
- Strong clinical judgment and patient-centered communication skills.
- Passion for helping patients maximize independence and quality of life.
- Home health experience is preferred but not required.
- Valid driver's license and reliable transportation.
