Full Job Description
Our team is in need of a Community Liaison for Orange County. This role would support Home Health, Hospice, and Homecare services.

Our ideal candidate will have an established referral source(s) in the area and built relationships in the community! If you have worked in a sales capacity for Home Health, Hospice, Homecare, Medical Device, Pharmaceuticals, etc we would like to talk to you!

Overview

We are seeking a dedicated and compassionate Community Liaison to serve as a vital link between our hospice/home health/homecare services and the community. The ideal candidate will possess strong communication and negotiation skills, with a deep understanding of healthcare, palliative care, and community outreach. As a Community Liaison, you will promote our programs, build relationships with healthcare providers and community organizations, and ensure patients and families receive comprehensive support. This role offers an opportunity to make a meaningful difference while demonstrating leadership in a dynamic healthcare environment.

Responsibilities
  • Develop and implement outreach strategies to educate the community about hospice/home health/homecare services and benefits.
  • Establish and maintain professional relationships with healthcare providers, hospitals, nursing homes, and community organizations to facilitate patient referrals.
  • Conduct informational sessions to explain hospice/home health/homecare care options
  • Negotiate agreements and coordinate with external partners to enhance service delivery and expand program reach.
  • Analyze referral patterns, community needs, and market trends to identify opportunities for program growth.
  • Lead efforts in organizing community events, health fairs, and educational seminars to raise awareness of hospice care.
  • Utilize organizational skills to manage multiple accounts efficiently while maintaining detailed documentation of outreach activities.
