Introduction Loudoun County is one of the fastest growing counties in the country, known for its excellent quality of life and strong economy while embracing its historic presence. Located 35 miles west of Washington DC, Loudoun County offers diverse cultural, recreational, and educational opportunities. The mission of The Health Department is to provide health services that enhance and ensure the health of all Loudoun County residents.
Job Summary The Loudoun County Health Department is seeking an engaged and community driven Community Health Worker (CHW) Supervisor to help launch a new Community Health Worker (CHW) program within our Population Health team. This is an exciting opportunity to shape a brand-new initiative from the ground up and to build and mentor a team of future Community Health Workers dedicated to advancing health equity and strengthening community wellbeing across our diverse county.
Grounded in culturally responsive public health practice, the Supervisor provides daily leadership and guidance to support high-quality community outreach, strong documentation, and effective workflows. While some activities may appear limited in scope, the position requires extensive preparation, logistical coordination, material assembly, and responsiveness to changing operational needs behind the scenes. The Division Manager will provide feedback based on implementation experience and operational needs and may direct modifications to processes or workflows to ensure alignment with Population Health program goals and service effectiveness.
The Supervisor also coordinates outreach efforts, strengthens community partnerships, and contributes to initiatives that expand access to services and address social determinants of health. Working closely with internal teams and community organizations, the role helps shape strategies that reflect local needs. This position carries direct supervisory responsibilities, including coaching, performance support, and hiring input, and invites a spirit of curiosity, collaboration, and community-centered innovation.
