Full Job Description
The Community Dementia Program Manager is responsible for providing high quality dementia specific assessment and programming as well as community education, resource acquisition and communication to qualified members of the community, displaying good skills in assessment, program planning, and individual treatment in the promotion of health in Senior Association and Club Memory members. - Interviews and assesses community members for dementia risk and appropriateness for Club Memory membership - Assesses psychosocial needs, coping mechanisms, family dynamics, strengths and risk factors to meet education, care partner skill building and coping skills for Club Memory members and their families. - Provides dementia specific programming in accordance with Club Memory guidelines, including conduct of group therapeutic activity sessions tailored to the needs and capabilities of dementia specific populations. Uses a variety of approaches such as support groups, exercise, music, relaxation, stress reduction, etc. - Develops, implements, and evaluates programs as needed to meet the resource, clinical, and psychosocial needs of members and the families. - Helps members find resources for plans of care and refers to community organizations and agencies when needed during the assessment and or club membership process and/or change in status. - Provides effective communication to the Sibley team for members and for the groups as a whole as needs occur. - Documents attendance, communications, and changes on a timely and consistent basis. - Participates in and provides in-service education on dementia to community members, families, professionals, and staff. - Advocates on behalf of Club Memory members and their families. - Provides emotional support and education on available resources and disease process to members and their families. - Works well in close collaboration with physicians and other health care personnel in member evaluation and treatment to further their understanding of significant social and emotional factors underlying member's health and ability to cope and respond to different situations. - Relates well and builds relationships with members, families, and staff. - Demonstrates above average communication skills, including but not limited to public speaking, interviewing, and assessment skills. - Assists in writing grant applications. - Procures and maintains necessary supplies and equipment for activities. - Assumes professional responsibility for growth and development, setting annual goals for knowledge expansion while maintaining clinical expertise. - Displays excellent diagnostic and report writing skills. Eucation and Experience: - Bachelor's Degree required. - Nurse, Social Worker, Counselor, Family Therapy or Recreation Therapy experience preferred. - Experience programming for and working with individuals in long term care setting preferred. Salary Range: Minimum 32.62/hour - Maximum 53.81/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
