As an Clinical Team Manager, you'll contribute to our success in the following ways:
  • Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are consistently met.
  • Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
  • Ensure timely start of care and ongoing compliance with applicable regulatory requirements.
  • Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
  • Conduct regular OASIS reviews to support accurate documentation, compliance, and quality patient care.
  • Provide guidance and coaching to direct care team members and assist with interviewing and hiring clinical staff.
  • Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.


What is Required?
  • Associate degree in a Nursing related field required
  • 2 years home care experience as a registered nurse in a Home Health or Hospice environment
  • 1 year of supervisory experience preferred
  • Current, unrestricted RN license valid for the state of work
  • Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $93,000 to $100,000 annually, based on the company's good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
