DescriptionBroad Statement of Responsibilities: This role requires coordination of treatment experiences for professionals entering RTC, PHP or IOP services or admitting for a 3 day evaluation. It includes coordination with client, referent, outside providers, and treatment team, participation in all staffings and organization of case management tasks.

Education & Licensure: This position requires a minimum of a master's degree and one of the following licensures (LPC/LPCC/LMHC, LPC, LPCC, or LMHC, LMFT, MSW, PsyD or PhD)
Experience: Minimum of 1 year of case management experience preferred; 2 year minimum clinical experience

• Experience treating SUD
• Experience treating Professionals
Special Skills & Knowledge: Candidates should have the following skills and attributes: must be able to work and communicate well with people; be adaptable, and accept changes quickly and easily; be willing to address conflict appropriately. Basic computer skills are required with Microsoft Office Suite and web-based products knowledge a bonus.

Position Duties and Responsibilities - Responsibilities include but are not limited to:Clinical Coordinator-(Professionals) - Job Description LodgeAssist Virtually with Professionals RTC/PHP Clients• Admissions
o Assist with pre-admissions and intake assessments for Professionals Track clients
o Coordinate date, time, source, resources required, anticipated LOS, and assessment profile with call center; manage daily tracker of where client is in the queue
o Present synopsis of admitting clients to Professionals team to advise on client needs and any professional responsibilities during LOS
o Anticipate arrival and assist with receiving referent information and distributing to Professionals team
o Communicate with referents about client arrival in a timely manner
• During Client Stay
o Ensure ROI for family, workplace, referent, etc. are completed
§ Assign and/ or make collateral calls and distribute collateral information to treatment team
§ Legal: Ensure ROIs for any necessary legal support are completed and support clients with any court continuances, virtual attendances, probation officers, attorneys, or documentation that needs to be done during LOS.
§ Medical: Ensure ROIs for PCP, Psychiatric providers, therapists are completed. Communicate with providers and offer to send admission and discharge reports. Prepare during admission to have appointments set for 2 weeks post discharge.
§ Connect with any EAPs, unions, or other referral sources to establish preference of communication for LOS as needed
o Support completion of FMLA/STD/LTD documentation as needed
o Support clients with personal concerns (i.e. paying bills, canceling appointments, communicating with referents) as needed
o Provide assessments for clients as needed
o Schedule and manage CEC consults with clients, promoting Virtual Pros and OP Pros for aftercare
§ Use CEC consults to assess clients' stage of change, needs for remainder of treatment, and barriers to continued care/ return to work
§ Communicate with treatment team about CEC consults
o Screen clients who are referred to the Professionals track
§ Review Professionals curriculum with clients
§ Assess clients' readiness and goals for participation in the Professionals track; make clinical recommendations for Professionals engagement
o Manage referent communication
§ Based on staffings, clinical notes, and medical notes, complete comprehensive weekly updates and send to referents
§ Coordinate meetings with referents throughout clients' stay
• Discharge/Transfer
o Assist Professionals team in completion of Brief Discharge Summary, providing consult on aftercare recommendations and return to work recommendations
§ Distribute Brief Discharge Summary to relevant parties (i.e. referents, outside providers)
§ Assist in coordination with telehealth admissions, including scheduling warm hand offs and ensuring completion of telehealth forms/assessments
• Attend Professionals COC meetings; manage Professionals Track census
• Attend Professionals staffing
o Track client progress, aftercare plans, return to work recommendations, etc.
o Offer clinical consult to Professionals team
o Support in resource gathering as needed
• See Lodge clients for individual therapy virtually when coverage is needed
o Complete clinical documentation and treatment plan review forms in a timely manner
Telehealth Professionals• Assisst with Professionals preadmissions assessments
• Assisst with Telehealth Professionals client onboarding and other Telehealth Professionals case management duties as needed
• As clinical licensure allows, carry individual caseload of Professionals track clients
o Complete Biopsychosocial assessment and treatment planning
o Complete timely documentation in EMR
o Discharge/Transfer communication; complete return to work and aftercare recommednations, send discharge summaries to relevant parties
o Communicate with referents
o Participate in staffing; offer comprehensive updates on client progress and support clinical team
Evaluations (Lodge, DAC, DFW, Virtual)
• Complete pre-admit assessments for evaluations
o Determine ancillary evaluation needs
o Obtain relevant releases of information
o Introduce evaluation procedures
• Manage evaluation scheduling
• Manage referent communication
• Create client schedule
• Act as client's main point of contact throughout evaluation process, communicating schedule and evaluation information to client prior to arrival and assisting client during evaluation process
• Coordinate toxicology testing
• Send psych testing links and self-assessments
o Proctor pscyh testing as needed
• Create evaluation templates
o Add in relevant data from referral source information/previous evals
• Ensure timely communication with team members
• Take notes in internal debrief and outbriefing
o add relevant information to evaluation summary and comprehensive evaluation
• Complete collateral calls and communicate collateral information to team
• Manage timely completion of evaluation summaries and comprehensive evaluations
o Complete clinical interpretation of self-assessments
o Complete clinical interpretations of collateral information, write collateral summaries
o Edit working copies of evaluation summaries and comprehensive evaluations
o Complete final evaluation revisions
o Distribute final copies to client and referents
• Complete BPS and clinical assessments as needed
Required SkillsMust have, or be able to obtain within 90 days, the computers skills necessary to complete online learning requirements for job specific competencies, access online forms and policies, complete online benefits enrollment, etc.
