Full Job Description
Due to continued growth in our census, VNA is looking for a Clinical Supervisor for our Home Health division.

Job Summary: The Clinical Supervisor directs and manages a multidisciplinary team of home health clinicians and support staff to ensure delivery of patient-centered, safe, and cost effective care in the least restrictive setting. Clinical Supervisors provide the environment where patients, families, and stakeholders are viewed as essential members of the team, costs are contained, and enhanced outcomes are achieved. Clinical Supervisors provide a fluid and dynamic practice that is most effective when it changes and adapts with the challenges of the healthcare system. Stewardship of the healthcare dollars, safe transitions of care, evaluating patient adherence, and consistent stakeholder communication are critical interventions that Clinical Supervisors employ, while maintaining a primary and consistent focus on quality of care and patient self-determination.

Status: Full-Time (exempt)

Responsibilities:
  • Provides direct supervision, performance evaluations and disciplinary action of all assigned staff. Obtains input from Program Managers and Department leaders where appropriate. Evaluates and ensures that competency and compliance requirements are met for assigned staff.
  • Monitors staff assignments, caseloads, and team productivity. Promotes efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement.
  • Develops and maintains a system of patient tracking and financial forecasting. Ensure OASIS review and coding results in appropriate reimbursement for the patient's clinical condition. Refers and discusses high utilization and problem prone cases with Director of Clinical Services and Quality Assurance Staff.
  • Provides the Director of Clinical Services with census, staffing, team productivity and other reports as requested. Identifies staffing patterns and needs of assigned team.
  • Works collaboratively and in support of colleagues throughout the agency.
  • Ensures staff orientation and development programs are enacted.

Education/Experience/Skills/Qualifications:
  • Active Registered Nurse licenses in California
  • Minimum 2 years of Home Health experience
  • 1 year supervisor role preferred
  • Strong communication, leadership, organizational, decision-making, and time-management skills
  • Current CPR certification

  • Current driver's license

The compensation range for this role is determined by factors such as location and required skills. The salary range provided reflects the lowest to highest amount we reasonably anticipate paying for this position at the time of posting. Compensation is influenced by labor market conditions and various factors, including but not limited to: prevailing minimum wage in the area, relevant education, experience, skills, geographic location, performance, travel requirements, sales or revenue targets, and business needs. It is not typical for a new hire to be offered the highest end of the salary range. Pay decisions are based on the unique circumstances of each candidate and position.
