The Clinical Services Manager develops and leads clinical resources in delivery of quality cost effective care to patients. In partnership with the ROM, drives reduction of admissions and readmissions through a focus on clinical quality and coaching. This position is the clinical leader for their designated market and is responsible for mentoring and coaching assigned teams of Integrated Care Nurses to achieve performance and outcome expectations, increase skills, and professional development
Roles and Responsibilities:
Clinical Coaching:
- Owns and drives clinical quality through direct RN observation and coaching in the following categories: care delivery, patient interactions, preparation and prioritization, plan of care development and follow through, workflow tool utilization, documentation, and escalation forums
- Responsible for ongoing assessment and coaching of clinical acumen in pursuit of decreasing patient admissions and readmissions
Team/Collaboration/P&P
- Participate in required meetings with stakeholders, including SDCS, DCS, ROMs, IKC Care team, DKC teammates, and nephrologists
- Attend Clinical Reviews to drive continuous improvement
- Consult with operators as needed to drive quality
- Knows and complies with DaVita policies and procedures in accordance with State and Federal regulations for patient care and services (CMS, HIPAA, OSHA, or other governing agencies); Standards necessary to maintain organizational accreditation (such as NCQA)
- Topic SME as assigned
Tools, Technology, & Workflow Proficiency
- Responsible for assessing proficiency and providing guidance as it relates to clinical applications and reports
- Accountable to assess and provide guidance related to clinical workflow and clinical coaching on patient prioritization, critical thinking, and BDPs that align with the model of care.
- Reinforce care delivery, clinical training materials, educational tools, and policies for teammates through delivery of monthly CSM updates and ad hoc as needed
Other duties as assigned
- Reviews escalated REM events. Provides necessary investigation and plan of correction.
Education and Experience
- Current RN license in state of practice
- Current driver's license with positive driving record required; ability to meet requirements of insurance coverage, required
- 4 - 6 years relevant experience
- Bachelors degree
- Basic computer skills and proficiency in MS Word, Outlook and
Excel - Demonstrated experience managing to data driven outcomes
and process improvement - Experience in healthcare performance coaching (preferred)
- Current CRP certification (preferred)
- Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
Travel: Up to 75% depending on market Nights/Weekends: May include overnight travel
The Salary Range for the role is $78,000.00 - $119,000.00 per year.
If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year New York Non-exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hour
Washington Exempt: $80,168.40/year Washington Non-exempt: Bellingham: $19.13/hour, Burien: $21.71/hour, Everette: $20.77/hour, Unincorporated King County: $20.82/hour, Renton: $21.57/hour, Seattle: $21.30/hour, Tukwila: $21.65/hour, Remainder of Washington state: $17.13/hour
