Full Job Description
The Registered Nurse implements, directs, supervises, and evaluates nursing care for participants enrolled in the Institute on Aging PACE program. This role participates in participant health assessments, develops comprehensive nursing treatment plans, coordinates care with the interdisciplinary team, and provides nursing services in participants' homes and at the Day Health Center.Specific Responsibilities
  • Participate in the health assessment and examination of applicants and participants.
  • Serve as a member of the Intake and Assessment Team.
  • Evaluate participant needs and contribute to the development of a comprehensive nursing treatment plan for each participant.
  • Provide and coordinate nursing care in participants' homes and at the Day Health Center.
  • Manage minor illnesses, injuries, and stable chronic illnesses according to established procedures.
  • Initiate preventive and rehabilitative nursing procedures as appropriate for participant care and safety.
  • Evaluate acute illnesses in collaboration with the participant's primary care provider.
  • Coordinate referrals and follow-up care with appropriate healthcare providers.
  • Teach, supervise, and counsel participants and family members regarding nursing care needs and related concerns in the home.
  • Consult with and provide education to health workers and Home Care staff as required.
  • Review participant charts to coordinate care and ensure medical plans are carried out.
  • Conduct nursing home or home visits when appropriate and as directed by the physician.
  • Provide coverage in the Primary Care Clinic, which operates Monday through Friday.
  • Work Saturdays on a rotating basis.
  • Perform other reasonably related duties as assigned.


Qualifications
Required
  • Current California Registered Nurse license.
  • At least two years of nursing experience at the Registered Nurse level.
  • Ability to work effectively as part of an interdisciplinary team.
  • Demonstrated initiative, imagination, resourcefulness, and flexibility.
  • Interest in and willingness to work in a multilingual and multicultural environment.
Preferred
  • Public Health Nurse Certificate.
  • Experience in public health nursing, acute care nursing, or working with older adults.
  • Computer literacy.
  • Fluency in Spanish, Russian, or Chinese.


Compensation
  • Annual salary range: $100,401 to $125,000
