Clinical Pharmacy Specialist - Ambulatory Care/Cardiology

Job Overview

As the Cardiology Clinic Pharmacist, you will work closely with our cardiology team to ensure that patients receive the appropriate medications, education, and counseling to manage their cardiovascular health. You will provide expert pharmaceutical care, assist with medication management, and collaborate with healthcare providers to optimize patient outcomes. This role combines clinical pharmacy expertise with a focus on cardiology, allowing you to make a direct impact on the lives of patients with heart disease and related conditions.

Key Responsibilities
  • Medication Management: Review and manage medication regimens for cardiology patients, ensuring safety, efficacy, and adherence to treatment protocols.
  • Patient Counseling: Educate patients on their medications, including proper use, side effects, and potential interactions, focusing on cardiovascular drugs such as antihypertensives, anticoagulants, and lipid-lowering agents.
  • Collaborative Care: Work closely with cardiologists, nurses, and other healthcare providers to create and adjust treatment plans tailored to individual patient needs.
  • Clinical Support: Monitor patient progress, recommend appropriate medication adjustments, and assist with interpreting lab results to support clinical decisions.
  • Education and Training: Provide ongoing education to staff and patients regarding new treatments, therapies, and guidelines in cardiovascular care.
  • Pharmacovigilance: Monitor for adverse drug reactions and interactions, reporting findings as necessary.
  • Patient Advocacy: Ensure that patients are well-informed about their medications, lifestyle modifications, and treatment options, encouraging adherence to prescribed regimens.

Work Schedule: Full Time; 80 hours biweekly. This position will be assigned to the cardiology offices therefore will have to commute between Little River, SC and Myrtle Beach, SC.

• Licenses/Certifications/Registrations/Education:
• Baccalaureate degree or PharmD Degree from an accredited school of pharmacy, required
• Current, unrestricted license to practice pharmacy in the state of South Carolina
