Qualifications

  • Pharmacy licensure in the State of Texas required.
  • Two years of experience in a hospital pharmacy preferred.
  • Knowledge of JCAHO, OSHA, Medicare/Medicaid standards.
  • Familiarity with Texas State Board of Pharmacy regulations.
  • Ability to advise on drug therapeutic value and usage.

Responsibilities

  • Provide clinical and distributive pharmacy services to patients from Neonatal to Adult.
  • Dispense and label medications according to established standards.
  • Select and review drug utilization for safety and efficacy.
  • Store prescription drugs and maintain accurate drug records.
  • Implement decisions from the Pharmacy and Therapeutics Committee as per the Hospital Formulary.
  • Consult on the hazards and uses of drugs and devices as required.

Full Job Description
POSITION SUMMARY/RESPONSIBILITIES
Provides clinical and distributive services for Neonatal through Adult patients in accordance with JCAHO, OSHA, Medicare/Medicaid standards and Texas State Board of Pharmacy Rules and Regulations. Dispenses and labels drugs or devices, selects drugs and reviews drug utilization, stores prescription drugs and devices and maintains drug records in a pharmacy. Advises or consults when necessary or required by law about therapeutic value, content, hazard, or use of drugs and devices, or offering or performing the services and transactions necessary to operate a pharmacy. Implements decisions set forth by the P&T Committee as written in the Hospital Formulary and complies with Health System policies and procedures with regard to the dispensing, maintaining and storing of drugs.

EDUCATION/EXPERIENCE
Pharmacy licensure in the State of Texas is required. Two years experience in a hospital pharmacy preferred.

LICENSURE
Has current, or is eligible for licensure in Texas.
