We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our multidisciplinary team and play a crucial role in ensuring safe, effective, and individualized medication therapy for our patients.

About the Role

As a Clinical Pharmacist, you will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. You'll provide expertise in drug therapy management, medication reconciliation, and patient education, ensuring our patients receive the highest standard of care.

In this role, you will:
  • Ensure the appropriateness of all elements of pharmaceutical care while adhering to policies and procedures.
  • Supervise supportive team members to maintain accuracy and efficiency in distributive services.
  • Serve as a drug information resource for TriHealth providers.
  • Engage in direct patient interactions, precept pharmacy residents and student pharmacists, and contribute to department projects in collaboration with Pharmacy Leadership.

Job Overview:

A TriHealth Clinical Pharmacist is responsible for providing optimal pharmaceutical care to all patients we serve through distributive and clinical functions. The Clinical Pharmacist collaborates with other health care providers to ensure appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department's clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services. Additional responsibilities include, but are not limited to serving as a drug information resource for TriHealth providers, direct patient interaction, precepting of pharmacy residents and/or student pharmacists, and contribution to department projects or initiatives in collaboration with Pharmacy Leadership

Job Responsibilities:
  • Ensures the 7 patient "rights" are respected: right patient, right dose, right route, right time, right drug with the right information and documentation. Reports medication errors and assists in assessing the risks and strategies to manage the risk. Ensures compliance with regulatory requirements and organizational policies and procedures. Provides a consistent process of patient care to ensure the safety of the patient's medication use. Assists in protocol development, implementation, and optimization. Undertakes projects that will improve the quality of patient care and promote cost effective use of medications.
  • Takes the lead in identifying opportunities and solutions in pharmacy process improvements. Applies known solutions to a variety of predictable problems. Assumes personal responsibility for professional development, particularly in the area of ongoing clinical education, and develops clinical pharmacy programs according to policies and regulations. Participates in relevant committees. Assists in orientation and training of new hires, and precepts pharmacy students and residents.
    Contributes to direct patient care through careful review of patient information, assessment and provision of appropriate pharmaceutical recommendations.
  • Promotes the correct and appropriate use of medications by maximizing the clinical effect, using the most effective treatment, minimizing the risk of treatment-induced adverse events, and minimizing the expenditures for pharmacological treatments. Communicates findings and recommendations to other health care professionals who are responsible for the patient's care. Ensures that issues are resolved before dispensing. Provides education/counseling to patients about drug therapies, and provides concise, timely responses.
  • Creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm, professional environment. Establishes effective collaborative working relationships and open and active communication with pharmacy leadership, physicians, nurses, pharmacists, and technicians. Clearly articulates ideas and thoughts through written and verbal communications.
  • Provides drug distribution skills include verification, order entry, clarification as needed, accuracy of checking, technician supervision, and timely delivery. Utilizes clinical knowledge and experience to resolve health problems through rational use of medications. Manages the drug inventory by adjusting par levels and avoids stock-outs and decreasing waste. Communicates with the Pharmacy Buyers to ensure supply matches' demand. Ensures complete, accurate, and timely documentation.
