Qualifications

Responsibilities

  • Abide by and enforce compliance and ethical standards within the pharmacy practice
  • Direct clinical activities and develop therapeutic programs collaboratively with healthcare providers
  • Perform drug utilization reviews and monitor for adverse drug reactions
  • Prepare formulary abstracts for review by the Medication Use Committee
  • Educate hospital staff on clinical pharmacy practices and coordinate in-service training
  • Serve as a resource for medical teams during rounds regarding medication issues
  • Document interactions and interventions for quality assurance processes.

Responsibilities

  • The Clinical Pharmacist professional responsibility within their scope of practice
    • Abide by and enforce all compliance requirements and policies and performs these responsibilities in an ethical manner consistent with the organization's values.
    • Adheres to hospital policies including confidentiality.
    • Requires regular and prompt attendance.
    • Works the assigned schedule including overtime and call as required.
  • The Clinical Pharmacist patient care/department responsibilities
    • Requires minimal supervision to safely perform essential functions.
    • Utilization of proper body mechanics when
      • Moving equipment supplies that is necessary to perform essential functions
    • Maintain and operate equipment
    • Report broken or malfunctioning equipment
    • Directs the clinical activities of the pharmacy
    • Develops and coordinates clinical functions based on most current clinical pharmacy practise
    • Performs consultation, develops and coordinates therapeutic programs with physicians and nurses.
    • Measures quality and appropriateness of drug utilization review and adverse drug reactions.
    • Prepares formulary abstracts for review by Medication Use Committee for possible inclusion in the formulary.
    • Coordinates specialized education and in-service for hospital personnel as required.
    • Revises the formulary on an ongoing basis for annual publication.
    • Rounds with medical teams to serve as a resource for medication as directed.
    • Performs genetal pharmacy duties as needed:
      • Files and dispenses prescriptions
      • Prepares and dispenses solutions
      • Compounds medications
    • Serves on hospital committees as assigned
    • Serves as a liaison for pharmacy students of affiliated schools
    • Reviews, facilitates, initiates investigational drug protocols.
    • Dispenses controlled substances as defined by the controlled substances permit
  • The Clinical Pharmacist communication responsibilities:
    • Communicates and uses appropriate customer relation skills with physicians. patients, families and healthcare team in person and via telephone.
    • Responds lo overhead pages
  • The Clinical Pharmacist documentation responsibilities:
    • Documents physician interventions for inclusion into department QA
  • The Clinical Pharmacist citizenship responsibilities:
    • Accepts and completes all duties positively and without conflict.
    • Cooperates, helps others and improves the performance of the department.
    • Completes all mandatory department educational and hospital requirements.
    • Maintains orderliness and cleanliness of work areas, equipment and supply areas.
    • Adheres to current Infection Control and Safety Standards.
    • Ability to perform Pharmacists functions deemed appropriate by the Chief Pharmacist.

Qualifications

  • Graduation from a recognized school of pharmacy with a Pharm D. degree and completion of a one year post-doctoral residency program Required OR

  • Pharm D. degree Preferred
  • RPH - Registered Pharmacist in the State of Alabama Required

  • Comparable combination of education and experience may substitute for the above requirements.
