About the Job

The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.

The responsibilities of the Clinical Pharmacist include, but are not limited to:

Responsibilities:

  • Understands the External Pharmacy Review requirements as outlined in the Quality Assessment and Performance Improvement (QAPI) Plan.
  • Reviews BCSC participants' drug regimens and profiles as needed.
  • Collaborates with the BCSC Medical Director to develop effective review processes, document findings, and provide prescriber education.
  • Reports findings to the Quality Assessment and Performance (QAPI) Committee on a quarterly basis.
  • Analyzes chart review findings to identify opportunities for physician education and outreach.
  • Ensures appropriate medication management and utilization through chart review analysis.
  • Applies chart review findings to clinical outcome-focused projects.
  • Works with BCSC's contracted Pharmacy Benefit Management (PBM) firm to ensure efficient and accurate administration of pharmacy benefits.
  • Assists BCSC's contracted pharmacy with accurate Prescription Drug Event (PDE) reporting to CMS to meet all Medicare Part D requirements.
  • Identifies cost-effective pricing for both brand and generic medications.
  • Reviews and integrates comprehensive utilization data from the PBM into the data warehouse.
  • Manages the pharmacy network to ensure efficiency and compliance.
  • Provides accurate and timely enrollment and disenrollment data to the pharmacy, contracted facilities, and staff.
  • Coordinates with the contracted pharmacy on medication management issues.
  • Ensures appropriate in-service training for the proper dispensing, storage, and administration of participant medications.
  • Reviews storage protocols and system requirements for clinic and emergency medication supplies at least semi-annually or as needed.
  • Provides oversight of the medication refill process.
  • Creates process efficiencies to improve clinical workflow related to medication administration and processing.
  • Collaborates with the Health Plan Specialist to complete semi-annual and quarterly Medicare Part D reports as needed.
  • Develops and updates policies and procedures as required.
  • Prepares for and attends BCSC Clinic staff meetings.
  • Assists with drug usage guideline decisions at medical staff meetings by preparing and presenting utilization trends.
  • Ensures compliance with and optimization of BCSC policies and procedures related to medications and biologics.
  • Works with the Quality Assurance Coordinator to complete regulatory reports and filings as required.
  • Researches and responds to drug information inquiries from participants, families, and medical and administrative staff.
  • Attends all assigned committee meetings.
  • Leads Pharmacy & Therapeutics (P&T) meetings on a quarterly basis.
  • Conducts monthly audits of Controlled House Stock Medication counts.
  • Completes home visits as needed for medication reconciliation and participant education.


Experience and Skills:

Degree and Diplomas: Must possess a Doctor of Pharmacy (PharmD) degree.

Certificates & Licenses: A current California registered pharmacist license is required. Employment is contingent upon TB screening and the successful completion of a health examination by a healthcare provider. A valid California driver's license is also required.

Experience: A minimum of 2-3 years of experience in a managed care clinical pharmacy setting, clinic operations, or completion of a pharmacy practice residency. At least one year of experience working with frail or elderly populations is required.

Skills & Aptitudes: Strong written and verbal communication skills are required. Proficiency in Microsoft Office and general computer literacy is essential. Accuracy, attention to detail, and consistent follow-through are required. Ability to work effectively within a multidisciplinary team in the BCSC model of care. Sensitivity to the health beliefs and needs of a diverse, multi-ethnic aging population. Analytical skills are preferred.
