Overview
  • Department: IV Solutions/Oncology
  • Schedule: Full Time Days Mon-Fri (start time 8/8:30am)
  • Facility: Prime Healthcare St Mary's Infusion Center
  • Location: Chicago, IL 60622

**Position will be working at our infusion sites in Chicago, Evanston and Skokie

Responsibilities

Under the supervision of the Director of Pharmacy and clinical coordinator (if applicable), coordinates the activities of the pharmacy department to provide the highest quality of pharmaceutical care to all patients served by the hospital, while ensuring patient safety. Conducts 50% or more of professional time in direct patient care activities including but not limited to interdisciplinary rounding, patient counseling, or participation in patient care committees.  Provision of direct patient care encompasses activities such as the direct observation and evaluation of the patient and his/her medication-related needs; the initiation, modification, or discontinuation of patient-specific pharmacotherapy; and the ongoing pharmacotherapeutic monitoring and follow—up of patients in collaboration with other health professionals. The scope of patient care provided may include: General Medicine/Surgical, Intensive Care Services, Obstetrics & Gynecology, Pediatrics, Emergency Care, Oncology, Peri-operative, Geriatrics and Outpatient Services.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

  • Current state Registered Pharmacist License.
  • B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
  • Completion of postgraduate clinical training (PGY-1) or > 3 years of qualifying experience required.
  • Postgraduate clinical training PGY-2 preferred.
  • Board certification in a Board of Pharmacy Specialties clinical specialty preferred.
  • BLS required.
  • ACLS preferred.
  • Experience in hospital pharmacy operations preferred.
  • Additional Preferences:

  • Oncology, infusion and/or sterile compounding experience
  • Able to work independently from clinical and operations standpoint
  • Comfortable talking to providers, making independent clinical decisions and troubleshooting day to day problems

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $120,494.40 to $174,532.80 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
