As a pivotal member of our multidisciplinary team, the Clinical Pharmacist delivers exceptional clinical pharmacy services in both acute care and residential care settings. Your role involves personalized treatment assessments, serving as the primary drug information resource, and offering expert advice to healthcare professionals. You'll meticulously monitor drug prescribing, distribution, and therapy to ensure their safe, effective, and economical use.

Embracing a patient-centered approach, you'll oversee ongoing drug therapy, assessing factors like effectiveness, compliance, adverse reactions, and interactions. Your responsibilities include maintaining accurate workload records, communicating vital findings and recommendations to the healthcare team, and ensuring meticulous documentation in patient health records or computerized systems. Additionally, you'll interpret medication orders, input them into the database, and dispense prescriptions in strict adherence to legal, professional, and departmental standards. This is a Permanent Full Time (1.00) opportunity, working Monday to Friday day shifts with weekends and stats off.

Qualfications
Current registration as a pharmacist with the College of Pharmacists of British Columbia;

Certification of completion of an Accredited Canadian Pharmacy Residency program (ACPR1); OR

A combination of at least one year of full time equivalent (FTE) recent relevant clinical pharmacy experience and recognized advanced education courses; OR

A combination of at least 3 years of full time equivalent (FTE) recent relevant clinical pharmacy experience and an equivalent combination of training, knowledge and experience;

Active membership in the Canadian Society of Hospital Pharmacists is encouraged.
