Delta Health is seeking a
Clinical Pharmacist to provide safe, high-quality inpatient and outpatient pharmacy services. The Pharmacist is responsible for medication preparation and dispensing, medication management, clinical support, and compliance with hospital, state, and federal regulations.
Key Responsibilities- Review and interpret medication orders and identify potential therapeutic issues.
- Prepare, compound, package, label, and dispense sterile and non-sterile medications.
- Supervise inpatient and outpatient medication preparation and dispensing.
- Monitor drug therapies and provide clinical recommendations and interventions.
- Perform medication reconciliation and discharge medication counseling.
- Conduct medication therapy management under approved collaborative practice agreements.
- Provide drug information and education to physicians, nurses, patients, and other healthcare professionals.
- Oversee pharmacy technician workflow and verify medication preparation, labeling, order entry, and charges.
- Maintain compliance with Colorado Board of Pharmacy, DEA, FDA, and hospital requirements, including controlled substance records and medication inspections.
- Participate in quality improvement, auditing, education, and other pharmacy initiatives.
- Maintain a safe, clean, and compliant pharmacy environment.
- Share on-call responsibilities with other pharmacists.
Qualifications- Bachelor of Science in Pharmacy (B.S. Pharmacy) required.
- Current Colorado Pharmacist (RPh) license required.
- Previous hospital pharmacy experience preferred.
- Hospital pharmacy residency preferred.
- Experience with unit-dose, IV admixture, TPN, and/or pharmacokinetics preferred.
