Qualifications

• Doctor of Pharmacy (PharmD) or Bachelor's degree in Pharmacy required.
• At least 1 year of hospital pharmacist experience required.
• Active Licensed Pharmacist in Virginia required.
• Basic Life Support (BLS) certification within 90 days of hire required.
• Preferred: PGY 1 and/or PGY 2 cardiology residency experience.

Responsibilities

• Interpret, prepare, and verify medication orders appropriately.
• Provide clinical consultation with cost-effective alternatives.
• Issue controlled substances in compliance with DEA and VA Board of Pharmacy regulations.
• Participate in medication formulary review and approval process.
• Monitor drug therapies for interactions, allergies, and appropriateness.
• Report adverse drug reactions promptly and accurately.
• Minimize non-formulary procurement and support substitutions.
• Supervise pharmacy support staff and participate in continuous quality improvement.
• Engage in decentralized rounding with clinical teams and respond to emergent alerts and codes as designated.
