Full Job Description
Position Summary: The Clinical Pharmacist is responsible for ensuring the safe and effective use of specialty medications through drug utilization reviews and comprehensive clinical management services. This position provides oversight of patient care activities performed by technicians.

Essential Duties/Responsibilities:

Provide oversight and clinical pharmaceutical care outlined in Patient Care Program

Prevent, detect, and resolve medication-related problems and make appropriate evidence-based, patient-centered medication therapy recommendations

Verify prescription to be dispensed contains the correct medication and labeling according to company and regulatory guidelines

Provide drug information and education to patients, healthcare providers, and other care team members

Identify high risk patients and provide documented clinical intervention

Assist patients with the initiation, coordination, and referral to external healthcare services when necessary

Counsel patients and caregivers on medication and disease management, safe administration measures, and side effect management strategies during established points in therapy and upon request

Perform verification of prescriptions for appropriateness and accuracy

Ensure a drug utilization review is completed with each prescription dispensed

Provide clinical intervention prospectively, concurrently and retrospectively

Actively participate in the development and implementation of performance improvement, quality improvement, and patient safety activities

Report medication errors, adverse drug reactions, and medication recalls appropriately

Participate in clinical program development, critical pathways, disease management, and drug utilization programs

Maintain knowledge of specialty medications and administration delivery systems

Demonstrate quality and safety in the medication distribution process

Educate internal staff on specialty medications

Provide in-service and/or education to healthcare professionals outside of the organization when required

Will assume all Pharmacist on Duty responsibilities in the absence of the Pharmacist in Charge

Other duties as assigned

Required Skills/Abilities:

Minimum 3 years' experience in clinical patient care management required; Pharm.D. with clinical rotation experience will be acceptable

A knowledgeable individual with appropriate industry experience, preferably in the healthcare or mail order/specialty pharmacy industry

An intelligent and articulate individual who can relate to patients and possesses excellent communication skills

Ability to mentor technicians and work with peers in a team effort

Ability to exercise considerable judgment, maintain confidentiality and act in a diplomatic manner

Demonstrated ability to manage multiple priorities and deadlines

Sound technical skills, analytical ability, good judgment and strong operational focus

A well-organized and self-directed individual who is able to work with minimal amount of supervision

Capability to efficiently complete tasks in a fast-paced environment

Ability to work extended hours, weekends and holidays pursuant with industry demands

Ability to work a rotating on-call shift

Education/Experience:

Bachelor's degree in pharmacy or PharmD

Active Pharmacist license in good standing with applicable State Board of Pharmacy

Proficient in Microsoft Office and industry related pharmacy software programs.

Preferred Skill/Abilities:

Experience or knowledge of compliance, regulatory and accreditation rules, regulations and standards.

Prior experience in Retail, Mail Order or Specialty Pharmacy, PBM, pharmaceutical or managed health care industry preferred.
