Full Job Description
The Department of Pharmacy at Mount Auburn Hospital is seeking a Clinical Pharmacist to join our team. This pharmacist will work to support pharmacy operations across a variety of patient populations. This position will operate on a predominantly Monday-Friday evening shift schedule, with rotating weekends/holidays, and day/overnight coverage as needed.

Pharmacists work closely with other health care practitioners to meet the various needs of our patients. Pharmacists review and approve medication orders, monitor drug therapies, and provide drug information. They improve the health of patients by providing pharmaceutical care to individual patients and by serving as a resource to physicians, nurses, and other health professionals. Pharmacists take on charge responsibility and supervise and direct support staff. Clinical Pharmacists must have the ability to work independently with minimal to no direct supervision. They must be able to communicate effectively, both orally and in writing. The Clinical Pharmacist assumes direct responsibility of medication distribution, IV admixtures, safety and efficacy of prescribed drug therapy in accordance with laws and regulations governing pharmacists and the practice of pharmacy by the appropriate state and federal agencies. The Clinical Pharmacist is responsible for the optimization of drug therapy by identifying, resolving, and preventing drug related problems and improving therapeutic outcomes through their work in drug distribution, centralized pharmacy operations, and/or in patient care rounds. The Clinical Pharmacist will participate in patient care rounds as required by their assigned shifts and are required to staff every fourth weekend.

ESSENTIAL DUTIES & RESPONSIBILITIES (including but not limited to):

· Practices in compliance with all applicable state and federal laws, regulations, and accrediting bodies (e.g., Board of Registration in Pharmacy, DPH, FDA, DEA).

· Adheres to and interprets institutional, departmental, and pharmacy policies and procedures, and implements them on assigned shifts.

· Maintains required licensure, certifications, credentials, and continuing education.

· Assumes operational responsibility for assigned pharmacy service areas and shift activities without direct supervision.

· Provides staffing coverage across pharmacy areas as trained and assigned.

· Upholds a culture of professionalism, respect, teamwork, and patient safety within the Pharmacy Department.

· Coordinates and supervises the work of pharmacists, technicians, students, residents, interns, and other supportive personnel.

· Participates in onboarding, orientation, training, mentorship, precepting, and performance evaluation of pharmacy staff and learners.

· Prepares, compounds, verifies, and dispenses medications (including sterile products, unit dose, bulk, and extemporaneous compounds) accurately and safely.

· Ensures the accuracy, sterility, and timeliness of all medication preparations and intravenous products.

· Maintains accurate records and controls for controlled substances, investigational drugs, and clinical research medications.

· Reviews, interprets, and verifies medication orders for appropriateness, accuracy, and patient-specific factors; clarifies discrepancies with prescribers.

· Provides prospective evaluation, monitoring, and optimization of drug therapy, including pharmacokinetic dosing and surveillance activities.

· Identifies, documents, resolves, and helps prevent medication-related problems, adverse drug events, and near misses.

· Documents and communicates clinical interventions, consults, and medication-related information to ensure continuity of care.

· Provides drug information and clinical consultation to physicians, nurses, and other healthcare professionals.

· Supports transitions of care through medication reconciliation, discharge counseling, and preparation/dispensing of ambulatory prescriptions.

· Participates in quality assurance, utilization review, drug use evaluations, and other departmental or institutional initiatives.

· Performs other duties as assigned.

QUALIFICATIONS:

· Bachelor of Science in Pharmacy (five-year degree program) from an accredited college of pharmacy

· licensure to practice in the Commonwealth of Massachusetts from the Board of Registration in Pharmacy.

· Must have good oral and interpersonal communication skills, to communicate effectively with members of other healthcare disciplines, and other Pharmacy staff.

· Must have extensive knowledge and ability in all areas of the department including: IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution.

· Must have comprehensive knowledge of Pharmacotherapy.

· Ability to work collaboratively as a member of the patient care team.

· Must be able to stand/walk for prolonged periods of time (Standing or walking approximately 70% of the time). Must be able to lift up to 25 lbs.

· Awareness of occasional exposure to infectious disease and to possible carcinogenic, mutagenic, and sensitizing medications. Minimal exposure to caustic agents, acids, and fumes.

 

Pay Range:

$120,640.00 USD - $162,364.80 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
