Location: Tulsa, OK

Facility: Ascension St. John's Medical Center

Department/Specialty: Pharmacy Ambulatory Clinic

Schedule: FT, Monday-Friday, 8am-430pm or 830am-5pm

How you'll make an impact in this role

  • Enhance Patient Outcomes: Pro-actively manage chronic diseases (e.g., diabetes, hypertension) to prevent complications and improve overall quality of life.
  • Optimize Pharmacotherapy: Perform comprehensive medication management, interpret labs, and safely adjust dosages under collaborative practice agreements.
  • Boost Medication Adherence: Identify financial or insurance barriers and provide targeted education to streamline therapy and ensure access.
  • Support the Care Team: Collaborate directly with providers to streamline workflows and ease provider burnout.

What minimum requirements you'll need

Licensure / Certification / Registration:

  • Pharmacist credentialed from the Oklahoma State Board of Pharmacy required.


Education:

  • Bachelor's or advanced degree in Pharmacy required.

What additional preferences we're seeking

  • Pharmacist licensure by the state of Kansas prior to or within 6 months of hire date preferred
  • Board Certified Ambulatory Care Pharmacist credential preferred
  • Completion of a PGY-2 Ambulatory Care Pharmacy Residency in lieu of Ambulatory Pharmacist experience
