Full Job Description
Penn Medicine is seeking a full-time, day shift, hybrid, Ambulatory Clinical Pharmacist to support its clinical practices in Downtown Philadelphia.

Summary:
  • The Clinical Pharmacist, Ambulatory Care provides clinical pharmacy services within ambulatory care settings through direct patient care, comprehensive medication management, specialty medication management, and collaborative drug therapy management. The pharmacist collaborates with patients, providers, and interdisciplinary healthcare teams to design, recommend, implement, monitor, evaluate, and document patient-specific pharmacotherapy plans that optimize clinical outcomes, medication safety, and cost-effective care.The Clinical Pharmacist supports evidence-based medication use through patient and provider education, medication therapy optimization, medication access support, therapeutic monitoring, and collaboration with medication access, specialty pharmacy, and retail pharmacy teams. The pharmacist participates in quality improvement activities and performs duties in accordance with organizational policies, regulatory requirements, and professional practice standards.

Responsibilities:
  • Designs, recommends, monitors, evaluates and documents patient-specific pharmacotherapy to optimize safe, effective, and cost-conscious medication use.
  • Provides concise, applicable and timely responses to drug information requests from healthcare providers and patients.
  • Provides medication counseling and education to patients, caregivers, and healthcare professionals.
  • Collaborates with physicians, advanced practice providers, nurses, pharmacists, and other healthcare professionals to optimize medication therapy and improve patient outcomes.
  • Monitors medication therapy for appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, drug interactions, and adherence to evidence-based standards of care.
  • Participates in the management of medical emergencies.
  • Collaborates with medication access, specialty pharmacy, retail pharmacy, and interdisciplinary healthcare teams to facilitate medication access, address coverage and affordability barriers, and support timely initiation and continuity of prescribed therapies.
  • Supports prior authorization, payer, financial assistance, and medication access activities as appropriate to facilitate patient access to prescribed therapies.
  • Participates in quality improvement initiatives, medication-use evaluations, and other departmental and organizational projects to improve patient care and medication-use processes.
  • Precepts or directs the activities of pharmacy technicians, residents, interns, students, and other support personnel as assigned.
  • Participates in the education and training of pharmacists, technicians, interns, students, and other healthcare professionals as assigned.
  • Ensures adherence to medication-use policies, accreditation standards, regulatory requirements, and organizational policies and procedures.
Credentials:
  • PA Pharmacist License (Required)
  • NJ Pharmacist License within 90 days of hire (Required)
  • Board of Pharmaceutical Specialties Certification in area of specialty practice (Preferred)
Education or Equivalent Experience:
  • Pharmacy Degree from an accredited college pf pharmacy (Required)
  • Successful completion of a PGY1 Pharmacy Residency OR 3 years as a licensed pharmacist in a health system pharmacy with >/= 50% of time spent providing direct patient care through clinical pharmacy services. (Required)
