Qualifications

• Registered Nurse licensed in New Hampshire with an Associate's Degree in Nursing, or a Master's Degree in Therapy or Social Work.
• Minimum of three years in acute care or post-acute care settings.
• Prior supervisory or management experience in a clinical context.
• Knowledgeable in Home Health and/or Hospice regulations and Medicare standards.
• Skilled in team leadership and interdisciplinary collaboration.
• Strong analytical, organizational, and communication skills.
• CPR certified with a valid driver's license.

Responsibilities

• Lead and supervise a clinical team including nurses and therapists.
• Facilitate interdisciplinary meetings for coordinated patient care.
• Guide case managers on patient care and documentation.
• Review and assess patient progress and discharge readiness.
• Monitor various clinical quality and productivity metrics.
• Collaborate with various departments for performance improvement.
• Ensure compliance with relevant healthcare standards and policies.
• Analyze operational data to enhance patient outcomes and staff performance.
• Assist in recruitment and onboarding of clinical team members.
• Manage staffing assignments and departmental resources.
