We are seeking a dedicated licensed Clinical Manager to oversee and coordinate the daily clinical operations of our Home Health & Hospice programs in Washington State . The licensed Clinical Manager is responsible for ensuring high-quality patient care, compliance with state and federal regulations, and efficient management of clinical staff . This position plays a key role in maintaining DOH (Washington State Department of Health) compliance, Medicare/Medicaid regulations, and accreditation standards while fostering a culture of excellence, teamwork, and compassionate care.

Clinical Oversight & Quality Assurance - Ensure all patient care services adhere to Washington State Department of Health (DOH) home care and hospice licensing regulations
- Maintain compliance with Medicare/Medicaid Conditions of Participation (CoPs) and accreditation standards (Joint Commission, CHAP, or ACHC)
- Oversee patient assessments, care planning, and documentation to ensure accuracy, quality, and compliance
- Implement and monitor quality assurance and performance improvement (QAPI) programs to enhance patient outcomes
- Stay current with Washington State's opioid prescribing laws, advanced directives regulations, and end-of-life care policies
Team Leadership & Staff Management - Supervise and support RNs, LPNs, therapists, social workers, chaplains, bereavement staff, volunteers, and home health and hospice aides
- Conduct staff training and competency evaluations per Washington State DOH guidelines
- Lead case conferences and interdisciplinary team meetings to promote continuity of care
- Ensure compliance with Washington's continuing education (CE) requirements for nursing professionals
- Build a culture of belonging through active use of the company's recognition and reward platform
- Create an engaged and high-performing team through adoption and visual support of the company's people strategies
Regulatory Compliance & Documentation - Ensure timely, accurate, and complete documentation in Electronic Health Records (EHR) in compliance with Washington State home health & hospice documentation standards
- Prepare for state DOH inspections, Medicare audits, and accreditation surveys
- Maintain compliance with Washington's POLST (Physician Orders for Life-Sustaining Treatment) and Advance Directive requirements
- Address and resolve patient grievances and complaints in accordance with Washington State Patient Rights regulations
Operational & Financial Efficiency - Manage staff scheduling and resource allocation to optimize care delivery while maintaining cost efficiency
- Work with leadership to ensure financial sustainability and proper utilization of Medicare and Medicaid reimbursements under Washington State guidelines
- Assist in the development of policies and procedures specific to home health and hospice services in Washington
Patient & Family Advocacy - Serve as a liaison between patients, families, caregivers, and community resources to ensure comprehensive support
- Educate patients and families on Washington's Death with Dignity Act , hospice eligibility criteria, and palliative care options
- Promote community outreach, patient education, and end-of-life planning initiatives
- Leverage Lean thinking and methodology to develop contingency plans and staff competency with service recovery
Our requirements and qualifications for success - Active, unrestricted RN (Registered Nurse) license in Washington State (WA DOH licensed) or out-of-state MSL
- Follow the Washington Nurse Practice Act
- If MSL, complete Suicide Prevention Training in advance of hire per state regulations
- If MSL, complete Washington State Board of Nursing (WABON) MSL demographic survey in advance of hire per state regulations
- Bachelor's degree in Nursing (BSN) required; Master's degree (MSN, Healthcare Administration, or related field) preferred
- Minimum of 2-4 years of experience in home health, hospice, or a related healthcare field
- Strong knowledge of Washington State DOH home health and hospice regulations, Medicare/Medicaid requirements, and accreditation standards
- Leadership experience with staff supervision, team management, and performance evaluations
- Proficiency in Electronic Health Records (EHR) and case management software
- Strong organizational, problem-solving, and decision-making skills
- Excellent communication and interpersonal skills with a focus on patient-centered care
- Experience with survey readiness, quality assurance, and performance improvement programs
- Knowledge of Washington Medicaid Home and Community-Based Services (HCBS) waiver programs
- Certification in Hospice & Palliative Nursing (CHPN), OASIS (Outcome and Assessment Information Set), or other relevant credentials
- Bilingual skills; preferred but not required
- Driving is an essential function and/or a legitimate business purpose of this position; therefore, a valid driver's license is required as a condition of employment
Physical, environmental, and mental requirements - Ability to give direct patient care, including participating in lifting, transferring, positioning, and assistance of residents with walking or in wheelchairs
- Able to lift 40lbs on a regular basis
- Ability to use a laptop, tablet or touch screen to document resident records
- Apply your physical and muscle strength to lift, push, pull or carry objects
- Ability to utilize your lower back and abdominal muscles to support part of the body repeatedly and continuously over time without fatiguing

Salary Range: $93,730 to $142,105.39 per year
