Clinical Manager - Home HealthHaggai Healthcare | Gilbert, ArizonaFull-Time | Leadership PositionAs our Clinical Manager, you will work closely with our Director of Clinical Services, Clinical Supervisors, therapy leadership, office team, and field clinicians to oversee the delivery of safe, compliant, high-quality patient care.
Your responsibilities will include:
- Provide leadership, support, coaching, and accountability to our clinical team
- Oversee day-to-day clinical operations and patient care delivery
- Support Clinical Supervisors and field clinicians in resolving patient and operational concerns
- Ensure timely and appropriate coordination of care across disciplines
- Monitor documentation, plans of care, physician orders, missed visits, and other clinical performance indicators
- Help improve patient experience and HHCAHPS performance
- Participate in QAPI initiatives and use data to identify opportunities for improvement
- Maintain compliance with CMS Conditions of Participation, Arizona requirements, CHAP standards, and Haggai policies
- Support OASIS accuracy, clinical documentation, and survey readiness
- Address patient and family concerns promptly and professionally
- Partner with staffing and operations to ensure patients receive timely care
- Develop clinicians through education, coaching, performance management, and recognition
- Help create systems that allow Haggai to continue growing without sacrificing quality or culture
Who We're Looking ForOur ideal candidate is an experienced home health professional who is equally comfortable
leading people, managing clinical operations, and improving processes.
You may be a great fit if you:
- Are a Registered Nurse (RN) with an active Arizona license
- Have previous Medicare-certified home health experience
- Have clinical leadership, supervisory, or management experience
- Understand OASIS, plans of care, care coordination, and home health documentation
- Have working knowledge of CMS Conditions of Participation and home health compliance requirements
- Are comfortable reviewing clinical and operational performance data
- Communicate clearly and professionally-even when the conversation is difficult
- Know how to coach employees while still holding them accountable
- Are organized, responsive, and capable of managing competing priorities
- Look for solutions instead of waiting for someone else to solve the problem
- Care deeply about both the patient experience and employee experience
Experience with
KanTime, CHAP accreditation, QAPI, HHCAHPS, and survey preparation is highly valued.
