Responsibilities for Clinical Liaison:
  • Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.
  • Promote the organization's philosophy, mission, and objectives both internally and externally, serving as a trusted representative of our care standards and values.
  • Attend and actively participate in facility meetings, calls, and in-services to support collaboration, education, and strong communication across care settings.
  • Demonstrate honesty, integrity, and professionalism in all interactions, and ensure the respectful handling of patient information and hospital property.
  • Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.
  • Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.
  • Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.
