Description

JOB DESCRIPTION

The Clinical Liaison serves as a trusted resource and consultant to community referral partners, promoting awareness of the Kin On's services. Acting as an advocate for all services, including skilled nursing care, assisted living, adult family home, home care, community wellness and social services, the Clinical Liaison builds strategic relationships, provides education to healthcare professionals, and connects individuals and families with appropriate care solutions. This role focuses on generating qualified referrals, building long-term relationships with hospitals, physicians, discharge planners, community organizations, and culturally diverse communities while promoting Kin On as the premier provider of culturally responsive senior care.

GENERAL RESPONSIBILITIES

Essential Duties and Responsibilities include the following. Other duties, responsibilities, and activities may change or be assigned at any time with or without notice.
  • Develop and maintain a strong Kin On presence within hospitals, physician practices, skilled nursing facilities, rehabilitation centers, adult family homes, senior living communities, home health agencies, community organizations, and other healthcare settings to promote awareness of and referrals to the full continuum of Kin On services.
  • Serve as a consultative resource to healthcare providers, case managers, discharge planners, social workers, payors, community partners, and referral sources regarding Kin On's services, including skilled nursing care, assisted living, adult family home, home care, community wellness and social services.
  • Educate healthcare professionals, referral sources, patients, and families regarding care options, eligibility criteria, clinical services, reimbursement guidelines, and the benefits of Kin On programs.
  • Build, strengthen, and maintain productive relationships with referral partners to support community awareness, patient access, and growth across all Kin On service lines.
  • Serve as an advocate for individuals and families by helping identify appropriate care options and facilitating access to Kin On services based on medical, functional, psychosocial, and personal needs.
  • Conduct assessments, screenings, and consultations, as appropriate, to determine program eligibility and identify the most appropriate level of care within the Kin On continuum.
  • Provide education to patients, families, and caregivers regarding available services, care transitions, treatment options, and available community resources.
  • Coordinate seamless transitions between healthcare settings and across Kin On programs, ensuring continuity of care and an exceptional customer experience.
  • Collaborate with physicians, nurses, therapists, social workers, discharge planners, case managers, and other healthcare professionals to support patient-centered care planning and successful transitions.
  • Facilitate referrals and admissions to appropriate Kin On service lines, ensuring compliance with organizational policies, regulatory requirements, and payer guidelines.
  • Assist patients and families in navigating healthcare systems, insurance coverage, Medicare, Medicaid, managed care plans, and other reimbursement sources as they relate to Kin On services.
  • Identify opportunities for internal referrals across Kin On programs and work collaboratively with operational leaders to coordinate smooth transitions between services.
  • Participate in community outreach activities, networking events, professional associations, educational programs, health fairs, and marketing initiatives to strengthen Kin On's visibility and reputation.
  • Monitor referral activity, admission trends, market opportunities, customer satisfaction, and other performance indicators; prepare and maintain reports, databases, and outcome metrics as required.
  • Collaborate with clinical and operational leaders to identify community needs, improve referral processes, and support strategic growth initiatives across all Kin On service lines.
  • Maintain current knowledge of healthcare regulations, reimbursement methodologies, industry trends, and competitive market factors affecting skilled nursing care, assisted living, adult family home, and home care.
  • Participate in interdisciplinary meetings, community planning efforts, and organizational initiatives to promote Kin On's mission, values, and commitment to excellence in care.
  • Perform other related duties as assigned to support organizational growth, customer service excellence, and the strategic objectives of Kin On.


QUALIFICATIONS,
  • Bachelor's degree in Healthcare Administration, Marketing, Business, Communications, Social Work, Gerontology, Public Health, Nursing, or a related field.
  • Three (3) years of progressive experience in healthcare marketing, community relations, business development, admissions, discharge planning, senior living sales, home health, hospice, post-acute care, or related healthcare services.
  • Demonstrated experience developing referral relationships with hospitals, physicians, skilled nursing facilities, case managers, social workers, senior living communities, and other healthcare providers.
  • Experience presenting educational programs and conducting community outreach activities.
  • Valid driver's license with reliable transportation for community outreach.
  • Candidate must be authorized to work in the U.S..


KNOWLEDGE AND SKILLS

  • Good working knowledge of MS Office Suite products such as Outlook, Word, Excel, PowerPoint, CRM and also of software and hardware used in patient care facilities and business lines.
  • Driving ability to operate a motor vehicle safely and legally. Ability to navigate and adhere to planned routes, utilizing maps or GPS devices. Maintain a clean driving record. Adequate vision and hearing to operate a vehicle safely.


COMPENSATION AND BENEFITS:
  • Pay: From $80,168.40 per year depending on experiences
