What You’ll Do As a trusted liaison between hospitals, healthcare providers, and our skilled nursing centers, you will:● Develop and strengthen relationships with hospitals, physicians, case managers, discharge planners, accountable care organizations, and community partners.● Identify new referral opportunities while maintaining and growing existing partnerships.● Coordinate patient referrals from initial clinical review through admission, ensuring seamless transitions of care.● Collaborate with admissions, nursing, and executive leadership to deliver outstanding patient and customer experiences.● Promote the clinical capabilities and specialty programs that set our centers apart.● Leverage CRM technology and market data to identify growth opportunities and measure performance.● Create and execute strategic business development plans that increase admissions and market share.● Analyze referral trends, conversion metrics, and market intelligence to continuously improve results. Preferred qualifications include:● RN or LPN license preferred (where applicable), but not required.● Valid driver’s license with the ability to travel locally.● Five or more years of clinical, healthcare sales, business development, case management, or post-acute care experience.● Experience working with hospitals, managed care organizations, or skilled nursing facilities is highly desirable.● Excellent communication, presentation, and relationship-building skills.● Ability to analyze data, develop strategic plans, and achieve measurable performance goals.
