$85,000 per year to build a program from the ground up. Nao Medical is launching a new outpatient substance use disorder program at our Mineola clinic, and we are looking for an LCSW to lead it. The role is a 50/50 blend of clinical practice and program leadership: set the clinical direction, supervise the team, grow enrollment through community outreach, and carry a psychotherapy caseload, with one day per week supporting our HealthySteps early childhood program in Canarsie. You will report directly to our CEO, work alongside physicians and NPs under the same roof, and use AI copilots so documentation never crowds out patient care.

What You'll Do- Clinical Practice: Biopsychosocial assessments, level-of-care determinations, and individual psychotherapy using CBT, DBT, motivational interviewing, relapse prevention, and harm reduction, plus crisis intervention and MAT coordination.
- Program Leadership: Set clinical direction, build workflows and documentation systems, and track enrollment, engagement, retention, and outcomes.
- Supervision: Clinical and administrative supervision, case review, and coaching for CASAC staff, peer advocates, and social workers pursuing licensure.
- Community Outreach: Build and track referral pipelines with schools, justice-involved settings, and community partners.
- Care Coordination: Own each care plan from intake through closure, screen for social determinants of health, and connect patients to housing, transportation, food, vocational, and mutual aid resources.
- HealthySteps (Canarsie): One day per week, conduct psychosocial assessments for children ages 0 to 3 and their caregivers during well-child visits. Assessment only, not pediatric therapy.
- Documentation and Compliance: Maintain EMR documentation that meets program standards and 42 CFR Part 2, and support utilization review and audit readiness.
- Team Collaboration: Partner with physicians, NPs, and care coordinators to integrate behavioral health with primary care.
Requirements- Master's Degree in Social Work (MSW) from an accredited institution
- Active New York State LCSW license (required)
- Experience providing substance use disorder treatment, including assessment, counseling, and treatment planning
- Experience supervising or leading clinical staff, or readiness to step into a supervisory role
- Demonstrated skill in CBT and DBT
- Strong working knowledge of New York community resources, Medicaid/Medicare, and social support systems
- Preferred: care coordination, SDOH screening, and population health experience
- Preferred: experience conducting psychosocial assessments with children
- Bilingual fluency in Spanish is highly preferred
