Responsibilities

  • Manage individual patient services and collaborate with clinical staff to develop rehabilitation plans.
  • Formulate transdisciplinary rehabilitation treatment plans in conjunction with patient and clinical team.
  • Complete evaluations and develop progress and discharge reports while monitoring patient status.
  • Lead interdisciplinary case conferences, communicating treatment plans and progress reports to stakeholders.
  • Facilitate acquisition of necessary equipment for patient functional goals with funding sources.
  • Monitor satisfaction levels of patients, families, and referral sources with the program.
  • Adhere to professional standards, HIPAA regulations, and maintain clinical practice skills.

  • The Clinical Coordinator (CC) serves as the central point of contact for patients, families, providers, and the interdisciplinary rehabilitation team. This role is multi-faceted and instrumental in creating a seamless, effective, and comprehensive rehabilitation experience for individuals with neurological diagnoses.
  • Our patients are seen in their homes and communities, allowing you to help them regain independence and successfully reintegrate into daily life where it matters most.
  • A0The Clinical Coordinator's (CC) role is multi-faceted, and supports a more efficient, effective and comprehensive transdisciplinary rehabilitation program similar to case managers utilized within inpatient rehab hospitals. The Clinical Coordinator acts as a team lead and is a central point of contact to coordinate, facilitate, orchestrate and in general ensure that a patient's rehabilitation and community reintegration needs are being met.

B7 Manages the provision of individual patient services and collaborates with clinical staff to develop rehabilitation plans. Coordinates the ongoing provision of rehabilitation services.B7 Formulates comprehensive, transdisciplinary rehabilitation treatment plans, clinical outcomes, goals/objectives and service delivery for referred patients in conjunction with the patient/care partner and the clinical team.B7 Completes CC evaluation and develops patient initial, progress and discharge reports and monitors the patient27s status on an ongoing basis. CCs are responsible for completion of patient clinical outcome tools based on diagnosis.B7 Monitors patient, family, physician and payer/case manager, referral source satisfaction with the program on a regular basis.B7 Leads interdisciplinary case conferences, team meetings, and communicates Treatment Plans/Progress Reports on a regular basis to the physicians, external case manager/payor and/or patient/family members.B7 Schedules/facilitates interdisciplinary case conferences with the clinical team, payers/case managers, physicians and family members.B7 Communicates regularly with patient, family/ care partner, payer/case manager and physician regarding patient progress and any important issues that arise.B7 Facilitates acquisition of necessary equipment to enable achievement of agreed upon functional goals (i.e. HMO, insurance carrier, etc.) with funding source.B7 Participates in medical provider meetings to report/resolve medical issues for person served.A0

B7 Monitors projected hours of service to be delivered to the patient and reports hours/subsequent changes to the local leadership (Executive Director/Director of Clinical Management/Program Manager).B7 Negotiates with funding sources for extensions of service beyond initial authorizations.B7 Acquires and tracks orders from physician for all provided ongoing services after the initial orders are obtained.B7 Adheres to standards of the Company's Code of Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.B7 Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy and regulations. Maintains and enhances clinical practice skills.B7 Performs other duties, as assigned.

Qualifications

B7 Minimum of Bachelor27s degree in Allied Health (OT, PT SLP), Registered Nurse, Human Services, orRehabilitation Counseling/PsychologyandB7 Maintains relevant professional licensure/registration/certification in the state of practice.B7 Current certification as Certified Brain Injury Specialist (CBIS) preferred.B7 Current BLS (Basic Life Support) First Aid and CPR certifications. B7 Minimum of two-year27s experience in a rehabilitation setting with neurologically impaired patientsB7 Prior case management and/or insurance administration experience preferred.B7 Demonstrates excellent organization, problem solving, decision making and communication skills.B7 Organized and able to function under minimal supervision.B7 Has reliable transportation and able to travel to/from multiple assignments/locations on a daily basis.

Additional Job Information

 Ability to lift fifty (50) pounds. Moving, lifting, or transferring of patients may involve lifting of up to 100 pounds. Ability to stand for extended periods. Fine motor skills Visual acuity Duties require extensive standing and walking. Requires lifting, positioning, pushing, and/or transferring patients. Duties require frequent reaching, stooping, bending, kneeling, and crouching.

Salary Range USD $95,680.00 - $108,160.00 / Year
