- Graduate of an occupational therapy assistant program accredited by the American Occupational Therapy Association.
- Certified with the National Certification Examination of the American Occupational Therapy Association.
- Licensed Certified Occupational Therapist Assistant by the Oregon State Board
- Preferred 1-2 years of clinical experience as a COTA.
- Current BLS for Healthcare Professionals.
- Must be a licensed driver with a reliable automobile.
