Full Job Description
Pathways PACE has an immediate opening for a Center Director based out of Greeley, Colorado.

LOCATION: 1900 16th Street, Greeley, CO

STATUS: Full-time

SCHEDULE: Monday-Friday, 8am-5pm

ANNUAL PAY RANGE: $109,553 - $128,710

SUPPLEMENTAL PAY: Based on position, schedule and/or availability: Paid Mileage/Shift Diff/Stipends/Overtime

MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile - one of the highest in the industry!

REWARDING WORK YOU WILL DO:

The purpose of this position is to oversee and administer the assigned Program of All inclusive Care for the Elderly (PACE) Center. This includes all aspects which consist of a primary care medical clinic, 24-hour care delivery, preventive services, a provider network, therapeutic services, a day health center, home care, transportation and performance improvement activities. Duties include ensuring compliance with Centers for Medicare and Medicaid Services' (CMS) and Colorado state PACE guidelines.

ESSENTIAL DUTIES:

People:
  • Accountable for the development, implementation, monitoring and continuous improvement of effective operations and teams, ensuring that the interdisciplinary needs of patients and families are met.
  • Uses effective interpersonal skills that promote interdisciplinary collaboration and communication throughout the organization.
  • Oversees the scheduling of adequate team members to meet patient/family needs including selection and training of new colleagues.
  • Supervises and maintains qualified team members, ensuring that appropriate personnel perform their functions within the organization, and team members and contract providers are properly trained on all policies and procedures.
  • Meets regularly with direct reports to review goals, trends and performance measures to celebrate successes and to facilitate change as needed.
  • Ensures that contracted providers are aware of and adhere to the organization's policies and procedures. Defines and informs team members of the relationship of PACE services to Colorado PACE.
  • Models attendance at Care Synergy and Colorado PACE events and in-services.

Service:
  • Ensures effective facilitation of interdisciplinary team (IDT) meetings for the coordination of activities, nutrition, personal care, clinic services, transportation, rehabilitation and other services.
  • Assists in the coordination of 24-hour care delivery.
  • Fulfills administrative requirements such as completion of forms as necessary, including daily participation reports, statistical records, and data forms, charts entries in a timely and accurate manner.
  • Performs all other reasonably related jobs as assigned.
  • Maintains a cooperative and supportive work relationship with participants, family and program team members.
  • Maintains the confidentiality of all Company procedures, results and information about participants, clients or families.

Quality
  • Ensures compliance with appropriate regulations and standards of care. Participates in survey processes.
  • Ensures compliance with policies and procedures.
  • Ensures effective communication with contracted providers to include education/orientation to PACE Programming.
  • Ensures that all regulatory reporting requirements are completed timely and accurately.
  • Ensures that participants are oriented to the program and have opportunities to express their opinions about the care and services they receive.
  • Ensures that all concerns/grievances are responded to in a timely manner.

Growth:
  • Completes annual mandatory education requirements.
  • Represents Colorado PACE at state and regional industry association meetings/conferences.
  • Develops partnerships within the community and educates community members, healthcare providers and educational institutions about Colorado PACE philosophy and services.
  • Empowers team members to develop community relationships and provides education on Colorado PACE Care services.
  • Collaborates with peers to move Colorado PACE toward its operational and strategic goals.

Finance:
  • Utilizes company resources in a prudent and appropriate manner. Is a good steward of company resources.
  • Utilizes problem solving skills required in conjunction with program team members with the goal of providing the highest level of quality care in the most cost-efficient model possible. Problem solving will be necessary to guide the care delivery team to the most appropriate decision that will meet both requirements of cost efficiency and quality.
  • Ensures the provision of quality service within expected standards and financial targets.
  • Works with the interdisciplinary team (IDT) to develop, monitor and maintain systems that assure optimal utilization of resources.
  • Develops and recommends annual percentages for operations and monitors services and ratios to stay within the Colorado PACE model as measured through per member per month benchmarking.

WHAT WE ARE GOING TO LOVE ABOUT YOU:
  • Minimum Education: Graduate of an accredited school of nursing; BSN preferred. In lieu of BSN, relevant experience may be considered.
  • Minimum Experience: Five (5) years progressive management experience with ten (10) years health care experience (hospice preferred).
  • Required License: Current Colorado RN license. Valid Colorado State driver's license and auto insurance. CPR certification required.

PHYSICAL REQUIREMENTS: Ability to lift/carry a minimum of 30 lbs.
